Zusammenfassung

Die in Teil I dieser Studie [Hessemer und Mitarb., Klin. Mbl. Augenheilk. 199 (1991) 346-355] phänomenologisch dargestellten okulären Kreislaufveränderungen bei Halothan-Lachgas-Narkose werden hinsichtlich ihrer Mechanismen analysiert. - Ergebnisse: Es bestand eine signifikant (P<0,05) lineare Relation zwischen dem Narkose-induzierten Abfall des systolisch-retinalen und -ziliaren Blutdrucks und dem Abfall des systolischen A.-brachialis-Drucks (ΔPs,bra). Die gleiche Beziehung bestand zwischen der Reduktion der systolischokulären Perfusionsdrucke (retinal und ziliar) und ΔPs,bra. Die Korrelationskoeffizienten lagen für die genannten Relationen zwischen 0,37 und 0,58. Die Beziehung zwischen der bei Narkose vorliegenden Augeninnendrucksenkung und ΔPs,bra war - zumindest für den Zeitraum 5-15 min nach Intubation - tendenziell linear (P=0,07); der Korrelationskoeffizient betrug 0,37. Die Reduktion des okulären Pulsationsvolumens (PVoc) und der pulsatilen okulären Durchblutung (Fp,oc) war nicht abhängig vom Systemblutdruck, jedoch vom arteriellen CO2-Partialdruck (PCO2): Mit sinkendem PCO2 - d.h. mit zunehmender Hyperventilation - fielen auch PVoc und Fp,oc ab. Die Relationen PVoc vs. PCO2 und Fp,oc vs. PCO2 waren signifikant linear (P<0,01), die Korrelationskoeffizienten lagen zwischen 0,58 und 0,7. Die weiteren okulären Meßgrößen korrelierten nicht mit dem PCO2 - Schlußfolgerungen: Die bei Halothan-Lachgas-Narkose vorliegenden Veränderungen okulärer Kreislaufparameter und des Augeninnendrucks sind partiell zurückzuführen auf die - überwiegend Halothan-bedingte - Reduktion von Systemblutdruck und Herzschlagvolumen sowie auf eine relative chorioidale Vasokonstriktion bei Hyperventilation. Quantitativ sind die genannten Faktoren allein jedoch nicht ausreichend zur vollständigen Erklärung der beobachteten Effekte. Als weitere Mechanismen kommen direkte lokale Narkotika-Einflüsse auf die okuläre und orbitale Mikrozirkulation sowie auf die augeninnendruckregulierenden Effektorstrukturen in Frage.

Summary

In part I of this study [Hessemer and associates, Klin. Mbl. Augenheilk. 199 (1991) 346-355] the phenomenology of the ocular circulatory changes, found during general anesthesia using halothane and nitrous oxide, has been described. These changes are analyzed with respect to their mechanisms. - Results: There was a significantly (P<0.05) linear relation between the anesthesia-induced reduction of the systolic ocular (retinal and ciliary) blood pressures and the decrease of the systolic brachial artery pressure (ΔPs,bra). The same relation was present between the reduction of the systolic ocular (retinal and ciliary) perfusion pressures and ΔPs,bra. The correlation coefficients for the designated relations ranged between 0.37 and 0.58. The relation between the anesthesia-induced intraocular pressure reduction and ΔPs,bra, was - in tendency - linear (P = 0.07), at least for the time period 5-15 min after intubation; the correlation coefficient amounted to 0.37. The reduction of both the ocular pulsation volume (PVoc) and the pulsatile ocular blood flow (Fp,oc) did not depend on the systemic blood pressure, but on the arterial CO2 tension (PCO2: Decreasing PCO2 - i.e., increasing hyperventilation - was paralleled by decreases of PVoc and Fp,oc. The relations PV oc vs. PCO2 and Fp,oc vs. PCO2 were significantly linear (P<0.01), the correlation coefficients ranged between 0.58 and 0.7. All other ocular variables did not correlate with the PCO2 - Conclusions: The changes of ocular circulatory variables and of the intraocular pressure, observed during halothane-nitrous oxide anesthesia, are partially accounted for by the - mostly halothane-induced - reduction of the systemic blood pressure and of the cardiac stroke volume as well as by a relative choroidal vasoconstriction occurring during hyperventilation. Quantitatively, however, the designated factors alone are not sufficient for complete explanation of the observed effects. Further mechanisms to be considered are direct local influences of general anesthetics on both the ocular and orbital microcirculation as well as on the intraocu lar pressure-regulating effector structures.
