Zusammenfassung
Dieser Literaturreview untersuchte die kortikalen Reorganisationsprozesse bei Patienten mit CRPS, welche Hirnareale von diesen Veränderungen betroffen sind und wie sich die Anpassungen zeigen. Weiterhin wurden die Zusammenhänge der Abläufe mit den klinischen Symptomen evaluiert. Es ergab sich Evidenz für die substanzielle Reorganisation der somatotopischen Karte innerhalb des Kortex. Von diesen Umstrukturierungsprozessen ist ein komplexes Netzwerk betroffen, das nicht nur bei nozizeptiven Reaktionen, sondern auch bei motorischen und kognitiven Prozessen involviert ist. Außerdem fanden sich eine signifikante Schrumpfung der kortikalen Handrepräsentation kontralateral zur betroffenen Hand auf dem primären somatosensorischen Kortex sowie eine Verschiebung der Handrepräsentation zur Lippe hin. Die einzigen klaren Anzeichen für ein CRPS sind der Schmerz und die Hyperalgesie.
Abstract
This literature review examined the cortical reorganisation processes in patients with CRPS, which cortex areas are affected by these changes and how the adaptations are presented. Furthermore, the coherence of these processes with the clinical symptoms was investigated. The results showed evidence for the substantial reorganisation of the somatotopical map within the cortex. A complex network is affected by the reorganisation processes. It is not only involved in nociceptive reaction but also in motor and cognitive processes. In addition, significant shrinking of the cortical hand representation contralaterally to the affected hand on the primary somatosensory cortex as well as displacement of the hand representation towards the lip was detected. Pain and hyperalgesia are the only obvious indications of CRPS.
