Angiosarkom des Os temporale[*]

Zusammenfassung

Das Angiosarkom des Felsenbeins ist ein äußerst seltener, bösartiger Tumor, der von den Gefäßendothelien ausgeht. Wir berichten über eine 57-jährige Patientin, die zunächst über eine progrediente Hörminderung klagte und im weiteren Verlauf eine Otorrhoe sowie eine Fazialisparese entwickelte. Die auswärtig mit dem Verdacht auf eine Mastoiditis durchgeführte erweiterte Antrotomie und eine Revisions-Operation im Sinne einer subtotalen Mastoidektomie ergab ein non-in-sano reseziertes Angiosarkom. Darauf folgend wurde bei einer Tumorausdehnung vom Mastoid über das Mittelohr bis zur temporobasalen Dura und nach lateral bis zur Subkutis eine subtotale Petrosektomie mit Durateilresektion durchgeführt. Postoperativ erhielt die Patientin eine Radiatio. Bereits nach wenigen Wochen zeigte die Patientin ein ausgedehntes lokoregionäres Rezidiv mit intrazerebraler Ausdehnung. Nach Einleitung einer Polychemotherapie in palliativer Intention nach dem Weichteilsarkomschema verstarb die Patientin an den Folgen einer Pneumonie im Rahmen einer Knochenmarksaplasie.

Abstract

Angiosarcoma of the temporal bone is an extremely rare malignant tumor, which originates from vascular endothelium. We describe a case of a 57-year-old woman, initially presenting with progressive hearing loss, otorrhea as well as facial palsy diagnosed as a mastoiditis elsewhere. After subtotal mastoidectomy histological examination revealed an angiosarcoma of the mastoid. Since the tumor was not completely removed lateral petrosectomy and postoperative radiotherapy were performed. Afterwards the patient developed local recurrence with intracerebral tumor extension. During palliative polychemotherapy the patient developed a pneumonia and deceased. In this manuscript the morphology, imaging characteristics and current treatment options of angiosarcomas of the lateral skull base are reviewed and discussed.
