Patientenzufriedenheit nach filtrierender Glaukomchirurgie mit Mitomycin C

Zusammenfassung

Hintergrund: Über die Patientenzufriedenheit nach filtrierender Glaukomchirurgie existieren nur wenige veröffentlichte Daten. Ziel unserer Studie war es, die Faktoren zu untersuchen, die die Zufriedenheit der Patienten mehr als ein Jahr postoperativ nach Trabekulektomie mit Mitomycin C beeinflussen. Material und Methoden: Ein zweiseitiger Fragebogen wurde im Rahmen einer prospektiven Studie mit der Bitte um anonyme Beantwortung an 61 weibliche, konsekutiv operierte Glaukompatienten gesandt, bei denen zwischen 2004 und 2005 ein filtrierender Eingriff mit Mitomycin C an der Universitätsaugenklinik Köln durchgeführt worden war. Ergebnisse: Die Antwortrate betrug 79 %, das mittlere Follow-up 19,2 ± 2,5 Monate. Nach Auswertung der Fragebogen fand sich keine signifikante Korrelation zwischen der Patientenzufriedenheit und den intraokularen Tensiowerten, Rötung des Auges, Empfindlichkeit des Auges und Fremdkörpergefühl. Eine signifikante Korrelation existierte zwischen der Patientenzufriedenheit und der Sehfunktion, eine signifikante inverse Korrelation zwischen der Zufriedenheit und der Anzahl der Besuche beim Augenarzt sowie der Tropffrequenz der Augenmedikation im Vergleich zu präoperativ. Eine schwach signifikante inverse Korrelation zeigte sich zwischen der Patientenzufriedenheit und der Anzahl der topischen Medikamente. Schlussfolgerung: Die Patientenzufriedenheit scheint stark von praktischen Folgen für das Alltagsleben abzuhängen, denn die Aspekte, die die Patientenzufriedenheit in unserer Studie am meisten beeinflussten, waren eine gute visuelle Funktion, geringere Frequenz der Augentropfenapplikation und geringere Anzahl der Besuche beim Augenarzt. Lokale Beschwerden aufgrund der Filterzone scheinen einen geringeren Einfluss auf die Patientenzufriedenheit zu haben als vermutet.

Abstract

Background: There are few published data on patient satisfaction following glaucoma surgery. The aim of our study was to investigate factors influencing patient satisfaction more than one year after trabeculectomy with mitomycin C. Material and Methods: We sent a two-page questionnaire to 61 female glaucoma patients who had consecutively undergone filtering surgery with MMC at the Department of Ophthalmology, University of Cologne, requesting anonymous replies (mean follow-up 19.2 ± 2.5 months). Results: The response rate was 79 %. There was no significant inverse correlation between patient satisfaction and recent IOP values, redness of the eye, local irritation or foreign body sensation. There was a significant correlation between patient satisfaction and visual function, a significant inverse correlation between frequency of visits to the ophthalmologist and frequency of eye drop application. A weak, insignificant inverse correlation was found between satisfaction and number of topical medications. Conclusions: Patient satisfaction seems to correlate primarily with practical impact on everyday life, visual function, frequency of eye drop application and visits to the eye doctor being identified as the major issues. Local symptoms related to the filtering bleb seem to have a minor influence on patient satisfaction.
