Differenzialdiagnostik der spastischen Spinalparalyse - Abgrenzung neurodegenerativer Erkrankungen gegen symptomatische Genese

Das Syndrom der progredienten spastischen Paraparese umfasst ein weites Spektrum an Erkrankungen unterschiedlicher Ätiologie. Hierzu gehören zum einen neurodegenerative Motoneuronerkrankungen wie die hereditäre spastische Paraparese oder die primäre Lateralsklerose, zum anderen Erkrankungen der weißen Substanz, mit z.B. autoimmunologischer und neurometabolischer Genese, zudem Infektionserkrankungen und Intoxikationen. Eine wesentliche, weitere ursächliche Erkrankungsgruppe stellen die strukturellen Pathologien des Rückenmarks dar.

Summary

The clinical syndrome of the progressive spastic paraparesis can be caused by a broad spectrum of various disorders, including neurodegenerative motor neuron disorders such as hereditary spastic paraparesis and primary lateral sclerosis. Other underlying etiologies are white matter diseases (of autoimmunological or neurometabolic origin), infections or intoxications. A further important group of causative pathologies are structural lesions of the myelon.
