Atem- und Sprechstörungen bei idiopathischem Parkinson-Syndrom und ihre Behandlung

Atem- sowie die Stimm- und Sprechstörungen bei idiopathischem Parkinson-Syndrom (IPS) werden in unterschiedlichen Bezügen beschrieben. Die Behandlung der rigid-hypokinetischen Dysarthrophonie wird anhand des evidenzbasierten Lee Silverman Voice Treatment (LSVT) erläutert - eine Intensivtherapie, die den Behandlungsschwerpunkt auf Lautstärke legt. Sie gilt in den Leitlinien der Deutschen Gesellschaft für Neurologie als Therapie-A-Empfehlung. Neben Verbesserungen der Lautstärke und Verständlichkeit wird auch die Sprechatmung quantitativ gesteigert. Klinisch hat sich die Atemtherapie nach Ilse Middendorf als sinnvolle Ergänzung zu LSVT und als qualitative Behandlung der Sprech- und der Ruheatmung erwiesen. Erste Beobachtungen und Ergebnisse der Kombination von LSVT mit Atemtherapie werden erstmals in diesem Zusammenhang dargestellt.

The respiratory dysfunctions and the speech disorders in parkinson's disease (PD) are presented from different points of view. The newest and generally perceived state-of-the-art treatment for the rigid-hypokinetic dysarthria is the Lee Silverman Voice Treatment (LSVT), judged as Level I evidenve for the efficacy of speech therapy in PD. LSVT can improve respiratory power in a quantitative way. To improve speechbreathing in a qualitative way and to treat also breathing at rest in PD, the Middendorf breathwork appears to be a useful addition. For the first time experiences and results of the combination of LSVT with Middendorf breathwork are described.
