Zusammenfassung
Die erektile Dysfunktion (ED) ist eine häufige altersabhängige Befindlichkeitsstörung, welche die männliche Lebensqualität empfindlich beeinträchtigen kann. Ihre Gesamtprävalenz über alle Altersgruppen beträgt in Deutschland rund 20 %. Entscheidend ist, dass Erektionsstörungen in der Regel als Symptom einer generellen endothelialen Dysfunktion zu verstehen sind. Typische Begleitmorbiditäten sind daher ein Diabetes mellitus, Depression und die koronare Herzkrankheit. Nach der diagnostischen Abklärung ist die orale Pharmakotherapie mit PDE-5-Inhibitoren die Therapie der Wahl - mit hoher Effektivität und einer geringen Nebenwirkungsrate. Spezifische Kontraindikationen sind natürlich zu beachten. Bleibt die orale medikamentöse Therapie erfolglos, werden vasoaktive Substanzen (Prostaglandin E1) oder Schwellkörperimplantate eingesetzt. Allerdings ist eine isolierte Betrachtung einer Erektionsstörung unter dem Aspekt „Männergesundheit” nicht sinnvoll. Zwar handelt es sich um ein spezifisches Männerproblem, vor allem jedoch um einen Frühmarker für physische und psychische männliche Gesundheitsprobleme.
Summary
Erectile dysfunction (ED) is a common, age-dependent functional symptom with a relevant impact on male life quality. The prevalence of erectile dysfunction in Germany is approximately 20 %. An important fact is, that erectile dysfunction must be understood as a symptom of general endothelial dysfunction. Typical comorbidities are diabetes mellitus, depression or coronary heart disease. The oral pharmacotherapy with PDE-5-inhibitors is the therapy of choice with a high efficacy and low side effects. Specific contraindications must be respected. Local pharmacotherapy with vasoactive substances (prostaglandin E1) or penile protheses are therapies in cases of failure of oral pharmacotherapy. The isolated view on erectile dysfunction does not make sense under the aspect of „men's health”. Erectile dysfunction is a specific male problem, but it is predominantly an early marker for organic and psychological health problems.
