Zusammenfassung

Die Psoriasis vulgaris ist eine der häufigsten dermatologischen Erkrankungen in der täglichen Praxis. Neben der Haut sind bei 5-10 % der Patienten auch die Gelenke im Sinne einer Psoriasis arthropatica befallen. Häufig sind Psoriasispatienten durch die auffallenden, manchmal doch sehr ausgeprägten Effloreszenzen psychisch stark beeinträchtigt. In den letzten Jahren sind durch ein besseres Verständnis von Ätiologie und Pathogenese der Erkrankung wichtige Fortschritte in der Therapie gelungen. So genannte Biologicals haben Einzug in die Behandlung der mittelschweren bis schweren Psoriasis gehalten und sind für viele Betroffene ein Meilenstein für ihre Lebensqualität. Dennoch sollten die bisher gut eingeführten und häufig ausreichenden Therapiemöglichkeiten nicht in Vergessenheit geraten, da bei diesen Verfahren viel Erfahrung bezüglich Wirksamkeit und Sicherheit vorliegt. Weiterhin sind bei der alltäglichen Therapie auch die Kosten für den behandelnden Arzt ein wichtiges Auswahlkriterium. Ein Überblick über den aktuellen Stand der Diagnostik und Therapie der Schuppenflechte wird in diesem Artikel gegeben.

Abstract

Psoriasis is a common inflammatory skin disease in general practice. Mild or moderate plaque psoriasis is the most important form. About 5-10 % of patients with psoriasis has psoriasis arthritis, a destructive joint disease. Often psoriasis has a negative effect on quality of life. Improved understanding of pathogenesis of psoriasis led to new systemic interventions of severe psoriasis. The biologicals can help to control skin lesions of psoriasis and are a milestone improving patients quality of life. Despite of this the old-fashioned therapy regimens are still important, because there is a broad experience with efficacy and safety of these interventions.
