TNFα-Antagonisten als therapeutische Bereicherung bei entzündlich rheumatischen Erkrankungen

Zusammenfassung

Bei der rheumatoiden Arthritis (RA) kommt TNFα in großer Menge in den befallenen Gelenken vor. TNFα-Antagonisten verbessern bei den meisten Patienten, die auf eine konventionelle antientzündliche Therapie nicht oder nicht mehr ansprechen, die körperliche Funktion und Lebensqualität. Darüber hinaus können sie das Fortschreiten struktureller Veränderungen an den gelenknahen Knochenanteilen bei der RA aufhalten. Gegenwärtig sind drei Biologika zur Blockade von TNFα verfügbar: Infliximab, Adalimumab und Etanercept. Bei Patienten mit RA wirken sie überzeugend und in der Regel recht schnell. Darüber wirken TNF-Blocker auch bei anderen entzündlich-rheumatischen Erkrankungen, besonders bei Psoriasisarthritis und Spondylitis ankylosans (M. Bechterew). Insgesamt ist die Therapie mit TNFα-Antagonisten gut verträglich, relevante Nebenwirkungen sind selten. Einschränkungen bestehen zurzeit durch das erhöhte Infektionsrisiko, noch unzureichende Langzeiterfahrungen und die noch relativ hohen Therapiekosten.

Summary

In rheumatoid arthritis (RA), TNF-alpha is found in considerable amounts in the joints affected. In the majority of the patients who do not, or no longer, respond to conventional anti-inflammatory therapy, TNF-alpha antagonists improve physical function and quality of life. In addition, in RA patients they can interrupt the progression of structural changes to the bones in proximity to the affected joints. Currently, three biologicals are available for the blockade of TNF-alpha: infliximab, adalimumab, and etanercept. In RA patients, they are impressively effective and usually are of rapid onset. In addition, TNF blockers also have a positive effect in other inflammatory rheumatic diseases, in particular psoriasis, arthritis and spondylitis ankylosans (AS, Bechterew's disease). Overall, treatment with TNF-alpha antagonists is well tolerated and relevant side effects are rare. Currently, certain reservations are prompted by the elevated infection risk, as yet insufficient long-term experience, and the still relatively high cost of treatment.
