Zusammenfassung

Schwieriges Asthma lässt sich definieren als Asthma, das sich trotz einer leitliniengerechten Therapie unter Einschluss von Kortikosteroiden und β-adrenergen Agonisten nicht ausreichend kontrollieren lässt. Viele assoziierte Faktoren wie Rhinosinusitis, gastroösophagealer Reflux, Allergenexposition, psychosoziale Faktoren und mangelnde Compliance können hierzu beitragen. Die Betreuung von Patienten mit schwierigem Asthma erfordert ein systematisches Vorgehen zur Sicherung der Diagnose und Behandlung. Aufgrund der klinischen Manifestation und der Entzündungsreaktion lassen sich verschiedene Subtypen unterscheiden. Neben der topischen Therapie mit Glukokortikosteroiden in Kombination mit langwirksamen β2-Sympathomimitika werden systemische Kortikosteroide oft in hohen Dosen zur Asthmakontrolle eingesetzt. Methotrexat, Cyclosporin A, α-Interferon, Anti-IgE haben bei einem Teil der Patienten einen kortikosteroidsparenden Effekt. Eine genaue Definition verschiedener Phänotypen kann zukünftig dazu beitragen unterschiedliche Genotypen mit variablem Ansprechen auf mögliche Behandlungsformen zu identifizieren.

Summary

Severe and difficult-to-treat asthma should be defined as asthma that remains uncontrolled despite the state of art use of asthma medication including corticosteroids and β-adrenergic agonist. Many associated factors such as rhinosinusitis, gastroesophageal reflux, persistent allergen exposure, psychosocial factors, and poor adherence to therapy can contribute to the severity of the disease. The care for patients with difficult-to-treat asthma requires a systematic approach for confirming the diagnosis and for treatment. Different subtypes can be distinguished by clinical manifestation and inflammation. High dose systemic steroids are often used in addition to inhaled steroids and long-acting β-agonist to control asthma. Adjuvant therapy with Methotrexat, Cyclosporin A, a-Interferon or Anti-IgE could have some steroid-sparing effect. A precise definition of asthma phenotypes is needed because different phenotypes may be associated with different genotypes, and different responses to treatment.
