Complications of Extracorporal Shockwave Therapy in the Treatment of Calcifying Tendinitis of the Shoulder

Fragestellung: Diese Studie untersuchte mögliche Nebenwirkungen der Stoßwellenbehandlung der Tendinitis calcarea. Bis jetzt hat keine Untersuchung die allgemeinen Nebenwirkungen dieses Verfahrens analysiert. Material und Methode: In dieser retrospektiven Studie wurden zwischen Jänner 2000 und April 2003 130 Patienten wegen symptomatischer Tendinitis calcarea mit extrakorporaler Stoßwellentherapie behandelt. Die Energieflussdichte lag stets im nieder- bis mittelenergetischen Bereich (0,012 - 0,2 mJ/mm²). Die Evaluierung unerwünschter Effekte wurde klinisch zur nachfolgenden Therapie und klinisch und radiologisch nach 6 und 12 Wochen durchgeführt. Auftreten, Dauer, Qualität und Intensität wurden detailliert registriert. Posttherapeutischer Schmerz wurde mit der visuellen Analogskala bewertet. Bei anhaltenden Beschwerden wurden erneut Röntgenbilder angefertigt und mit dem klinischen Zustandsbild in Verbindung mit dem Constant and Murley Score korreliert. Ergebnisse: In 97 % der Population konnte unmittelbar nach Therapie ein lokales Erythem beobachtet werden. 10 von 130 Patienten beklagten Schmerzen nach erfolgter Therapie. Andere Nebenwirkungen, besonders ossäre Veränderungen konnten nicht beobachtet werden. Schlussfolgerung: Nieder- und mittelenergetische Stoßwellentherapie scheint ein sicheres Verfahren in der Therapie der Tendinitis calcarea zu sein.

Abstract

Objective: Study investigates complications of the application of shockwaves in the treatment of shoulder joint disorders. To our knowledge, no one has investigated side effects of this treatment modality at the shoulder. Design: In this retrospective study, 130 patients were treated with extracorporal shock wave therapy for symptomatic calcifying tendinitis between January 2000 and April 2003. The energy flux density applied was in the low or mid-energetic range (0.012 - 0.2 mJ/mm²). The evaluation of side effects and treatment complications was performed clinically during each treatment session, and clinically and radiologically 6 and 12 weeks after the end of therapy, respectively. Clinical assessment comprised patient-reaction for complications, pain assessment using Visual Analogue Scales and the assessment of the Constant and Murley Score. In cases of persisting pain after therapy additional radiographs were made. Results: Local erythema at the interface of skin and lithotripter occurred in 97 % of the population. Pain due to lithotriptic therapy was observed in 10 of 130 patients. No other side effects, especially no bony alterations were observed. Conclusion: Low and mid-energetic shockwave therapy for calcifying tendinitis of the shoulder appear to be a safe conservative treatment modality.
