Biliäre Mikrohamartome der Leber - eine seltene Differentialdiagnose zur Lebermetastasierung

Zusammenfassung

Multiple biliäre Mikrohamartome der Leber (von Meyenburg-Komplexe) werden meist als seltene Zufallsbefunde bei Obduktionen diagnostiziert. Die 79-jährige Patientin wurde uns mit superinfiziertem Pagetkarzinom der rechten Mamille eingewiesen. In der Sonographie zeigte sich das Leberparenchym sehr inhomogen mit reflexreichen und reflexarmen Arealen, der Verdacht auf diffuse Lebermetastasierung wurde geäußert. Der sonographische und CT-morphologische Verdacht einer diffusen Lebermetastasierung bestätigte sich in der Feinnadelbiopsie nicht. Histologisch entsprachen die Veränderungen benignen biliären Mikrohamartomen, die aus Zysten und Bindegewebe bestehen und eine seltene Differenzialdiagnose darstellen. Besonderer Stellenwert in der Abklärung kommt der Magnetresonanztomographie zu, die in der T2-Wichtung eine Abgrenzung zur diffusen Lebermetastasierung ermöglicht [2] [3].

Abstract

Multiple bile duct hamartomas of the liver (von Meyenburg complex) are mostly incidental findings at autopsy. The 79-year-old female patient was presented to us with a superinfected Paget lesion of the right nipple. Ultrasound examination of the liver revealed multiple scattered hyperechoic lesions as well as some anechoic lesions, suspicious of metastatic disease. Computed tomography showed multiple focal lesions. The ultrasound-guided needle biopsy of the liver revealed multiple benign bile duct hamartamas. Magnetic resonance imaging ruled out diffuse malignant infiltration of the liver.
