Zusammenfassung
Ein 22-jähriger, bis anhin gesunder Mann bemerkte plötzlich ein Taubheitsgefühl und
ein Kribbeln in der linken Hand. Zwei Tage später trat ein intensiver Schmerz im linken
Arm 3 cm proximal zum Epicondylus medialis auf. Eine Woche später klagte er über ein
brennendes Gefühl in den Fingern I - IV volar. In der klinischen Untersuchung ergab
sich eine sensomotorische Medianusläsion. In den elektrophysiologischen Untersuchungen
konnte eine Läsion des Medianushauptstamms nachgewiesen und ein Karpaltunnelsyndrom
ausgeschlossen werden. Ein Tumor oder eine andere kompressive Läsion konnte mittels
MRI ausgeschlossen werden. Serologische Untersuchungen auf Vaskulitis oder infektiöse
Erkrankungen waren unauffällig. Der Schmerz und die sensomotorischen Defizite bildeten
sich innerhalb von zwei Monaten vollständig zurück. Wir stellten die Diagnose einer
spontanen Medianushauptstammläsion. Eine seltene Manifestation einer neuralgischen
Schulteramyotrophie stellt die wahrscheinlichste nosologische Zuordnung dar.
Abstract
A 22-year old previously healthy man noticed suddenly numbness and tingling in the
left hand. Two days later he felt an intense pain in the arm 3 cm proximal of the
medial epicondyle. A week later he complained about a burning sensation in the palmar
left fingers I - IV. On clinical examination we could observe a sensorimotor median
nerve deficit. Electrophysiological studies confirmed a lesion of the median nerve
trunc and excluded a carpal tunnel syndrome. A tumor or compressive lesion could be
excluded by MRI. Serological studies for vasculitis or infectious diseases were normal.
The pain and sensorimotor deficits subsided within 2 months. We conclude that the
present spontaneous median nerve trunc lesion is probably a rare manifestation of
a neuralgic amyotrophy.
