Zusammenfassung

Mit den akuten vorübergehenden psychotischen Störungen (AVPS) wurde in der ICD-10 eine neue Diagnosekategorie eingeführt, welche historisch auf Konzepte wie die schizophreniformen Psychosen, die reaktiven Psychosen und die zykloiden Psychosen zurückgeht. Die konstituierenden Merkmale der AVPS (ICD-10: F23) sind ein akuter Beginn sowie eine Dauer der Symptomatik von höchstens ein bis drei Monaten. Die bisherigen empirischen Untersuchungen weisen darauf hin, dass sich die AVPS aufgrund ihrer relativ günstigen Prognose im Hinblick auf den Langzeitverlauf klar von der Schizophrenie abgrenzen lassen. Die Diagnosestabilität der AVPS im weiteren Verlauf ist jedoch als eher gering einzuschätzen, wobei ein häufiger Diagnosewechsel zu den affektiven Störungen auffällt. Bezüglich der Therapie der AVPS gibt es bisher nur wenige empirische Daten, im Allgemeinen wird eine syndromorientierte Behandlung empfohlen.

Abstract

Based on historical concepts like schizophreniform psychoses, reactive psychoses and cycloid psychoses, the category „acute and transient psychotic disorders” (ATPD) was introduced as a new diagnostic concept in the ICD-10. The defining clinical features of ATPD (ICD-10: F23) are an acute onset and a duration of psychotic symptoms not exceeding one to three months. Previous empirical investigations showed that it is possible to separate ATPD from schizophrenia because of its more favourable prognosis with respect to the long-term course. However, diagnostic stability in the further course of ATPD seems to be low, whereby a frequent diagnostic change to affective disorders was observed. With regard to therapy, as yet only a small empirical database exists. Generally, a syndromatic treatment is recommended.
