Zusammenfassung

In der Literatur sind nur wenige Fälle von hypoglykämischer Neuropathie beschrieben; keine davon betraf den Bereich der Hirnnerven. Wir beschreiben den Fall einer 56-jährigen Patientin, die aufgrund eines Insulinoms seit Jahren an einem primären Hyperinsulinismus leidet. Als Nebenbefund besteht Zustand nach papillärem Schilddrüsenkarzinom. Die Hypoglykämien treten seit ca. 10 Jahren zunehmend und vermehrt auf und erreichen Werte von 20 mg/dl. Zuletzt entwickelte die Patientin eine Abduzensparese unklarer Genese, wobei im Artikel versucht wird, in Anlehnung an das Auftreten peripherer Polyneuropathien bei Hypoglykämien, eine Verbindung zu den rezidivierenden und massiv ausgeprägten hypoglykämischen Zuständen herzustellen.

Abstract

Only few cases of hypoglycaemic neuropathy are described in literature so far. None of these affected the cranial nerves. We describe a patient, who suffered from primary hyperinsulinism by a probable insulinoma for years. The hypoglycaemiae have increasingly occurred for 10 years and reach values of 20 mg/dl. Finally, the patient developed an abducens paresis of an unclear genesis. The article is trying to lean on the occurrence of peripheral polyneuropathies to establish a connection between the relapsing and very distinct hypoglycaemic states.
