Zusammenfassung

Die Knorpel-Knochen-Transplantation in „Pressfit”-Technik ist ein seit über 10 Jahren angewandtes Verfahren zur Behandlung von Gelenkknorpelschäden. Eine Nachuntersuchung an 20 Patienten zeigt ein gutes bis sehr gutes klinisches Ergebnis. Nachteile der Methode sehen wir in der limitierten Donorsite, den Defekten nach Transplantatentnahme und der nicht immer möglichen anatomisch korrekten Wiederherstellung der Gelenkflächenkonvexität. Wir sehen die Indikation für dieses Verfahren in der Behandlung von umschriebenen Knorpeldefekten Typ III und IV nach Outerbridge in der Belastungszone des Kniegelenkes.

The osteochondral transplantation. A suitable method of treatment of profound cartilage defects

Summary

For over ten years, the osteochondral transplantation in “press-fit” technique has been used in the treatment of articular cartilage defects. A follow-up in twenty patients showed good to very good clinical results. We see disadvantages of the method in the limited donor site, the defects after transplant harvesting and because it is not always possible to reconstruct the joint surface to be anatomically perfectly correct. We consider the osteochondral transplantation to be indicated in the treatment of located defects type III and IV according to the Outerbridge classification in the weight-bearing zone of the knee joint.

Résumé

La transplantation ostéo-cartilagineuse selon la technique «Pressfitt» est un procédé utilisé depuis plus de dix ans dans le traitement des lésions cartilagineuses. L'examen de contrôle de 20 patients montre de bons et très bons résultats cliniques.
Les inconvénients de la méthode sont les sites donneurs limités, les defects après prélèvements des transplants et l'impossibilité fréquente de rétablir une convexité anatomique correcte des surfaces articulaires.
Les indications de cette méthode sont pour nous les defects cartilagineux du type III et IV après outerbridge de la surface portante de l'articulation du genou.

Resumen

Por más de diez años el transplante osteocondral con técnica de „press-fit” ha sido usado en el tratamiento de los defectos del cartílago articular. Un seguimiento de veinte pacientes ha mostrado resultados clínicos buenos y muy buenos. Hemos visto desventajas en el método con respecto a lo limitado de la zona dadora, a los defectos luego de la toma de injerto y porque no es siempre posible reconstruir la superficie articular en forma anatómicamente perfecta. Consideramos que el transplante osteocondral está indicado en el tratamiento de defectos localizados tipo III y IV de acuerdo con la clasificación de Outerbridge en la zona de apoyo de la articulación de la rodilla.
