Die Behandlung instabiler kindlicher Oberschenkelschaftfrakturen mit elastisch-stabilen Verriegelungsnägeln

Zusammenfassung
Die elastisch-stabile Marknagelung (ESMN) ist eine etablierte Methode in der Kindertraumatologie. Nicht alle instabilen Frakturformen können mittels ESMN belastungsstabil versorgt werden. Zwischen 1995 und 1997 wurden 17 Kinder nach Oberschenkelfrakturen mit modifizierten Kinderverriegelungsnägeln belastungsstabil versorgt. Die Nachuntersuchung erfolgte nach 6 bis 32 Monaten. Klinisch fanden sich keine relevanten Beinlängendifferenzen, einmal zeigte sich ein Bruch der Verriegelungsschraube.Diese Behandlungsform stellt eine Alternative zur Fixateur externe-Behandlung dar und ist besonders kinderfreundlich.
Elastic stable interlocking nailing for femoral fractures in children
Summary
Many femoral fracture patterns in children cannot be stabilized sufficiently by intramedullary nailing only. Such fractures may require additional cast bracing or cerclage wiring after nailing. To overcome this problem pediatric flexible nails which can be locked were designed. Seventeen children from 2.5 to 15 years were treated with this method for unstable traumatic fractures of the femur. Average follow-up was 11.8 months. All fractures healed within four weeks in the mean. There were no major complications. This method prevents shortening and axial deviation. Start of postoperative mobilization and increase of weight-bearing is mainly determined by the child.
