Der problematische obere Atemweg beim Neugeborenen – glottische und subglottische Stenosen

Auffällige Atemgeräusche im Säuglingsalter sind durch Engstellen der Luftpassage bedingt. Die Geräusche lassen auch für geübte Ohren keine eindeutigen Rückschlüsse auf Art und Lokalisation der Ursache zu. Bei laryngotrachealen Stenosen ist das Ziel immer, eine Tracheotomie zu vermeiden – leider ist sie oft als Ultima Ratio unverzichtbar. Der Beitrag fokussiert die Behandlung von laryngotrachealen Stenosen im 1. Lebensjahr.

Kernaussagen
  • Die Behandlung von glottischen und subglottischen Atemwegsstenosen erfordert spezielle Kenntnisse und ein erfahrenes multidisziplinäres Team.

  • Für nahezu alle Formen der laryngotrachealen Stenosen stehen heute differenzierte Therapieverfahren zur Verfügung, die eine funktionelle Rehabilitation bzw. ein Dekanulement für nahezu alle Patienten erlauben.

  • Gerade bei Säuglingen ist die Anlage eines Tracheostomas mit einer nicht unerheblichen Morbidität und Mortalität verbunden.

  • Tracheotomievermeidende Interventionen stehen bereits im 1. Lebensjahr in einem breiten Spektrum zur Verfügung.

  • Die Kenntnis dieser Innovationen und der enge Austausch mit Kinderärzten/Neonatologen und Intensivtherapeuten sind die wesentlichen Voraussetzungen, dass möglichst viele betroffene Kinder primär einer adäquaten Behandlung in spezialisierten interdisziplinären Zentren für pädiatrische Atemwegerkrankungen zugeführt werden.
