Omental actinomycosis imitating a foreign body infection

Abstract

Actinomycosis is a rare chronic disease caused by a group of anaerobic Gram-positive bacteria that colonize the mouth, colon, and urogenital tract. Infection involving the cervicofacial area is the most common clinical presentation, followed by that of the pelvic and thoracic areas. The preoperative diagnosis of abdominal actinomycosis is difficult because the condition can be easily confused with malignancy, tuberculosis, or other inflammatory diseases. We report a case of omental actinomycosis mimicking a foreign body. Actinomycosis is diagnosed based on the histopathologic demonstration of sulfur granules in a tissue or organ that was surgically removed. Following confirmative diagnosis by surgical intervention, the patient was treated with oral doxycycline 100 mg twice daily for 6 months. A follow-up abdominopelvic computed tomography after 16 months showed no evidence of inflammation. Moreover, the patient no longer had disease-associated symptoms and exhibited normal laboratory findings.

Zusammenfassung

Actinomykose ist eine seltene chronische Erkrankung, die durch eine Gruppe von anaeroben grampositiven Bakterien verursacht wird, die sich im Mund, im Dickdarm und im Urogenitaltrakt ansiedeln. Eine Infektion des Zervikofazialbereichs ist die häufigste klinische Darstellung, gefolgt vom Becken- und Thoraxbereich. Eine präoperative Diagnose der abdominalen Actinomykose ist nicht einfach, da die Beschwerden leicht mit denen von Malignität, Tuberkulose oder anderen entzündlichen Erkrankungen zu verwechseln sind. Wir berichten über einen Fall von omentaler Aktinomykose, der einen Fremdkörper nachahmt. Actinomykose wird auf der Grundlage eines histopathologischen Nachweises von Schwefelgranulaten im Gewebe oder Organ diagnostiziert, die chirurgisch entfernt wurden. Nach der bestätigten Diagnose, die anhand eines chirurgischen Eingriff gestellt wurde, wurde der Patient mit oralem Doxycyclin 100 mg zweimal täglich über 6 Monate hinweg behandelt. Eine weiterführende abdominopelvische Computertomografie (CT) nach 16 Monaten zeigte keine Anzeichen einer Entzündung. Außerdem hatte der Patient keine weiteren Symptome, die mit der Erkrankung assoziiert werden und auch die Laborbefunde waren normal.
