Introduction

Scleritis is a serious inflammatory disease which is often associated with systemic autoimmune diseases such as granulomatosis with polyangiitis or rheumatoid arthritis [1]. Scleritis often is the first symptom to present in connective tissue diseases [1], [2]. In HIV positive patients, infections such as syphilis [3], tuberculosis [4] or toxoplasmosis are considered as the classical causes of scleritis.

To the best of our knowledge this is the first case report of severe posterior scleritis in a HIV positive patients, whom presented with none of the classical risk factors (i.e. syphilis, tuberculosis and toxoplasmosis were negative). Moreover, this case the severe scleral inflammation was also associated with superior serous retinal detachment which to date has rarely been reported [5], [6], [7].
