Die Arthrose des distalen Radioulnargelenks (DRUG) kann zu einer erheblichen schmerzhaften Funktionseinschränkung des Unterarms und der Hand führen. Dieser Beitrag gibt einen Überblick über die möglichen Ursachen der Arthrose und die operativen Behandlungsoptionen, die abhängig von den Ansprüchen des Patienten sowie der Erfahrung und Präferenz des Behandlers zu wählen sind.

Kernaussagen
  • Bei der operativen Primärtherapie der schmerzhaften Arthrose des distalen Radioulnargelenks (DRUG) stellen die Hemiresektions-Interpositions-Arthroplastik des Ulnakopfs nach Bowers und die Sauvé-Kapandji-Operation heute die am häufigsten verwendeten Standardverfahren dar.

  • Die Implantation einer Ulnakopfprothese als Primärverfahren sollte ausgewählten Patienten vorbehalten bleiben, da die Rückzugsmöglichkeiten beim Scheitern dieses Verfahrens begrenzt sind.

  • Die Implantation einer Ulnakopfprothese stellt immer dann eine gute Option dar, wenn es nach vorangegangener Resektions-Arthroplastik bzw. Sauvé-Kapandji-Operation zu einer schmerzhaften Instabilität des Ulnastumpfs kommt.

  • Der Komplettersatz des DRUG stellt eine weitere Rückzugsoption dar, insbesondere bei Scheitern einer Ulnakopfprothese bzw. in den Fällen, in denen eine ausreichende Weichteilstabilisierung am DRUG nicht gewährleistet ist.

  • Eine radioulnare Fusion kann als selten anzuwendende, letzte Behandlungsoption bei verbleibenden Schmerzen angesehen werden, nachdem alle anderen möglichen Optionen zuvor fehlgeschlagen sind.
