Why Not Endonasal Rhinoplasty?

This edition of Facial Plastic Surgery is dedicated to the endonasal rhinoplasty, with a review of its philosophy, discussion of important principles, and illustrating several key operative maneuvers. The terms “treading lightly,” “leaving a small footprint,” “protecting,” and “preserving the things you care about” are generally considered noble goals for any endeavor. As you will see, these tenets of conservation are evident in endonasal operation. This is a classic, time-honored operation that aims to optimize both nasal respiration and aesthetics. The surgeon consciously aims to preserve tissues and structures that are required for a successful outcome. The authors clearly describe how to maximally preserve soft tissue and support structure while maintaining a soft tissue envelope allowing for “real-time” adjustments of the nasal units. As you will see, the authors demonstrate how the endonasal approach permits decreased surgical time and minimizes soft tissue contracture during the healing phase. The endonasal rhinoplasty is embraced and employed by master surgeons around the world.
