Zusammenfassung

Proximale Femurfrakturen sind typische Frakturen des älteren Menschen. Aufgrund des zunehmenden Anteils älterer Menschen an der Bevölkerung ist künftigdeutlich häufigermit proximalenFemurfrakturen zu rechnen. Gleichzeitig stellen die betroffenen Patienten aufgrund ihres Alters und der damit häufig einhergehenden Multimorbidität sowohl aus chirurgischer als auch aus anästhesiologischer Sicht spezielle Anforderungen an die Behandlung. So spielen neben der operativen Versorgung mit dem Ziel der postoperativen Vollbelastung auch das interdisziplinäre perioperative Management sowie die frühestmögliche Einleitung geriatrisch-rehabilitativer Maßnahmen eine entscheidende Rolle.

Abstract

Hip fractures are among the most common fractures in elderly people. The annual number of femoral fractures is even expected to increase because of an aging society.

Due to the high number of comorbidities, there are special challenges in treating geriatric hip fracture patients, which require a multidisciplinarymanagement. This includes surgical treatment allowing full weight bearing in the immediate postoperative period, osteoporosis treatment and falls prevention as well as an early ortho-geriatric rehabilitation program.

  • Proximale Femurfrakturen sind typische Frakturen des alten Patienten.

  • Die beiden wichtigsten Risikofaktoren sind Osteoporose und Sturzneigung.

  • Häufig liegt eine Multimorbidität vor.

  • Zu den proximalen Femurfrakturen gehören Schenkelhalsfrakturen sowie per- und subtrochantäre Femurfrakturen.

  • In den allermeisten Fällen ist eine operative Therapie erforderlich, deren Ziel die postoperative Vollbelastung zur Vermeidung immobilitätsbedingter Komplikationen ist.

  • Die Operation sollte innerhalb der ersten 24 h nach dem Unfall erfolgen, da anschließend die Morbiditäts- und Mortalitätsrate steigt.

  • Im 1. postoperativen Jahr nach proximaler Femurfraktur verstirbt etwa ein Drittel der Patienten.

  • Wichtige positive Einflussfaktoren sind die interdisziplinäre Zusammenarbeit mit einer geriatrischen Abteilung sowie die frühestmögliche Einleitung geriatrisch-rehabilitativer Maßnahmen schon während der akutstationären Behandlung oder im direkten Anschluss.
