Biochemische Marker der Gerinnungsaktivierung und der Fibrinbildung als Risikoindikatoren

Zusammenfassung

In den letzten Jahren entwickelte biochemische Marker der Thrombin- und Fibrinbildung erlauben eine sensitive Diagnostik geringgradiger Gerinnungsaktivierungen. Normale Spiegel des fibrinspezifischen Spaltprodukts D-Dimer (ELISA) können zum Ausschluß einer frischen Thromboembolie dienen und damit diesen Patienten eine Phlebographie ersparen. Die Sensitivität nimmt mit dem Thrombosealter (über 7 Tage) und bei begrenzter Ausdehnung (Wadenvenenthrombose) ab. Der D-Dimer-Latex-Test ist für diesen Zweck nicht sensitiv genug. Zur Risikoabschätzung einer postoperativen Venenthrombose sind Mehrfachkontrollen als Verlaufsbeobachtung notwendig. Zur frühzeitigen Diagnostik der DIC ist insbesondere der Thrombin-Antithrombin-Ill-Komplex (TAT) geeignet. Die DIC kann damit zu einem Zeitpunkt diagnostiziert werden, zu dem noch erfolgversprechende therapeutische Möglichkeiten gegeben sind. Die Entwicklung von Schnellbestimmungen ist hier besonders wichtig. Unter einer zytostatischen Tumortherapie kann das Ansteigen dieser Parameter eine Thromboseneigung anzeigen. Weiter sind insbesondere TAT und das Prothrombinfragment (F1+2) zur Therapiekontrolle der Antikoagulation geeignet.

Insgesamt wird sich der Kliniker in seiner Entscheidung nicht ausschließlich auf einen Laborparameter verlassen können. Dennoch dürften diese Parameter in Zukunft für den Kliniker eine wichtige zusätzliche Entscheidungshilfe zur Risikoabschätzung bei bestimmten manifesten oder drohenden thromboembolischen Ereignissen geben.
