Zusammenfassung

Gegenstand: In früheren Studien wurde die Eignung des isoliert perfundierten Rindereuters zur Untersuchung der Antibiotikaverteilung im gesunden laktierenden Drüsengewebe nach intrazisternaler Gabe belegt, allerdings sind diese Resultate nicht einfach auf entzündete oder trockenstehende Drüsenkomplexe übertragbar. Material und Methoden: In der vorliegenden In-vitro-Studie wurde mit demselben Modell die Gewebsverteilung von Benzylpenicillin nach lokaler Applikation vergleichend in gesunden, entzündeten und trockenstehenden Vierteln gemessen. Ergebnisse: Die Verteilung des Wirkstoffs in den gesunden laktierenden Eutervierteln entsprach früher erzielten Ergebnissen. Bei den gesunden laktierenden wie auch bei den trockenstehenden oder entzündeten Drüsenvierteln verringerte sich die Arzneimittelkonzentration grundsätzlich mit zunehmender vertikaler Entfernung zur Zitze. Bei chronisch katarrhalischen Mastitiden mit interstitiellen Fibrosen unterschied sich die Gewebsverteilung von den gesunden Eutervierteln kaum. In gesunden trockenstehenden Vierteln wurden vor allem im zitzennahen Bereich wesentlich niedrigere Benzylpenicillin-Konzentrationen gemessen. Bei Vorliegen einer akuten Mastitis fielen die Wirkstoffkonzentrationen im Drüsengewebe in Abhängigkeit vom Abstand zur Zitze hoch exponentiell unter minimale Hemmkonzentrationen ab. Schlussfolgerungen: Mithilfe des isoliert perfundierten Rindereuters wurden erstmals Untersuchungen durchgeführt, die Daten für die Hypothese liefern, dass die Gewebsverteilung von lokal in das Euter applizierten Antibiotika durch Laktationsstadium, Gesundheitszustand und Entzündungsart des Euters beeinflusst werden. Klinische Relevanz: Bei Wahl der Applikationsform ist immer die Pathogenese des Einzelfalls im Auge zu behalten und insbesondere bei akuten Mastitiden zusätzlich eine systemische Behandlung in Erwägung zu ziehen, um eine ausreichende Hemmkonzentration im Drüsengewebe zu erreichen.

Summary

Objective:In previous studies, the suitability of the isolated perfused bovine udder to examine the distribution of antibiotics in healthy lactating glandular tissue after intracisternal administration was shown, but these results can not simply be transferred to inflamed or dried off mammary glands. Material and methods: In the present in vitro experiments using the same model, the tissue distribution of benzylpenicillin after local administration was measured comparatively in healthy, inflamed and dried off udder quarters. Results: The distribution pattern of the active constituent in the healthy quarters corresponded to results obtained before. In healthy lactating as well as in dried off or inflamed glandular quarters, in principle the drug concentration was reduced with increasing vertical distance to the teat. In chronic catarrhalic mastitis with interstitial fibrosis, the tissue distribution hardly differed from healthy udder quarters. In healthy dried off quarters, much lower benzylpenicillin concentrations were found, especially near the teats. In cases of acute mastitis, the concentration of the active constituent in the glandular tissue decreased highly exponentially with increasing distance to the teat below minimal inhibitory concentrations. Conclusions: With the isolated perfused bovine udder, for the first time tests were performed to substantiate the hypothesis that the tissue distribution of antibiotics administered topically into the udder is influenced by the status of lactation and health of the udder as well as by the type of inflammation. Clinical relevance: When choosing the route of administration, attention always has to be paid to the pathogenesis of the individual case and especially with acute mastitis, an additional systemic treatment has to be considered to achieve sufficient inhibitory concentrations in the glandular tissue.
