Aneurysma der V. poplitea mit Lungenembolie

Zusammenfassung

Venöse Aneurysmen sind selten und können Lungenembolien verursachen. Wir berichten über ein Aneurysma der V. poplitea als Ursache von Lungenembolien bei einer 53jährigen Patientin. Das Aneurysma wurde mittels Farbduplexsonographie und Phlebographie diagnostiziert. Die chirurgische Behandlung erfolgte durch Venenraffung und Thrombektomie. Postoperativ entwickelte die Patientin eine Peroneuslähmung und eine revisionsbedürftige Nachblutung. Venöse Aneurysmen mit thrombembolischen Komplikationen stellen eine strenge Indikation zur operativen Behandlung dar. Die operative Therapie eines Aneurysmas der V. poplitea kann problematisch sein und Komplikationen verursachen.

Summary

Venous aneurysms are rare and can cause pulmonary embolism. We report one case with popliteal vein aneurysm as a source of pulmonary embolism in a 53-yearold woman. The aneurysm was diagnosed by phlebography and color duplex ultrasound. We performed surgical treatment by aneurysm resection and thrombectomy. Postoperatively the patient showed peroneal nerve paralysis and developed big haematoma in the leg with need for surgical correction. Venous aneurysms with a history of thrombembolic complications will be a strict indication for operative therapy. Surgical management of popliteal venous aneurysm may lead to complications. Les anévrismes veineux sont rares et peuvent être à l´origine d´une embolie pulmonaire. Nous décrivons le cas d´une patiente de 53 ans présentant un anévrisme de la veine poplitée ayant provoqué une embolie pulmonaire. L´anévrisme a été diagnostiqué grâce à une échographie duplex en couleur et une phlébographie. Le traitement chirurgical a consisté en une vénorraphie et une thrombéctomie. Une révision a été nécessaire en raison d´une hémorragie au deuxième jour postopératoire, qui a entrainé une lésion du nerf sciatique poplité externe. Les anévrismes veineux avec des complications thromboemboliques sont soumis a des indications opératoires sévères. La cure chirurgicale d´un anévrisme de la veine poplitée est délicate et peut induire des complications. Anévrisme de la veine poplitée ayant provoqué une embolie pulmonaire
