Hamatumfrakturen – Diagnostik und Therapie

Zusammenfassung

Hamatumfrakturen werden in Frakturen des Hamulus (Typ 1) und Frakturen des Corpus ossis hamati (Typ 2) eingeteilt. Bei den Korpusfrakturen werden koronale Abrissfrakturen im Rahmen von karpometakarpalen (CMC-)Luxationen (Typ 2a) und Querfrakturen des Hamatums (Typ 2b) unterschieden.

Frische unverschobene Hamulusfrakturen (Typ 1) können konservativ behandelt werden mit Ruhigstellung des Handgelenks in 15° Flexion und 5° Radialduktion, bei allerdings unsicheren Ergebnissen. Die operative Therapie besteht entweder in einer Schraubenosteosynthese (direkt über offenen Zugang von palmar oder indirekt perkutan von dorsal) oder in einer Exstirpation des Hamulusfragments. Bei beiden Verfahren ist der R. profundus n. ulnaris unbedingt zu schonen, der ulnar um den Hamulus herum nach distal in die Tiefe zieht.

Bei koronalen Frakturen (Typ 2a) steht die Therapie der CMC-Luxation ganz im Vordergrund, die meist mit geschlossener Reposition und perkutaner Kirschner-Draht-Transfixation gelingt. Größere Hamatumfragmente können manchmal mit Draht oder Schraube refixiert werden.

Querfrakturen des Hamatums (Typ 2b) werden analog zu Skaphoidfrakturen durch Reposition und Osteosynthese therapiert, wenn sie als einzelne Verletzung vorliegen. Bei komplexen karpalen Frakturen sollte das Hamatum ungefähr anatomisch ausgerichtet und stabilisiert werden.
