Zusammenfassung

Die Therapie von Dystonien zielt auf die lokale Wirkung von Botulinumtoxinen (Btx) an der injizierten Muskulatur. In einigen Fällen wird dies jedoch von unerwünschten Effekten, u. a. auf das autonome Nervensystem begleitet. Um diese Effekte zu messen, wurden 36 Personen mittels autonomer Messungen u. a. des Pupillenlichtreflexes, der Tränensekretion und des sympathischen Hautreflexes untersucht. Verglichen wurden mit Botulinumtoxin behandelte Pa­tienten (n=20) mit fokalen Dystonien (cervicale Dystonie, Blepharospasmus) und Hemispasmus facialis vor und nach Btx-Injektion sowie mit Kontrollpersonen (n=16). Patienten wiesen im Vergleich zu den Kontrollprobanden signifikant verlängerte Reaktionszeiten, Konstriktionszeiten und Redilatationszeiten des Pupillenlichtreflexes und eine geringere Tränensekretion auf. Keine Unterschiede fanden wir beim Vergleich vor und nach Btx-Behandlung sowie hinsichtlich des sympathischen Hautreflexes. Die Ergebnisse belegen eine systemische Wirkung lokal injizierten Botulinumtoxins auf die autonome Steuerung des Pupillenlichtreflexes und der Tränensekretion, die bei den bereits länger vorbehandelten Patienten über die muskulären Effekte hinaus andauern und daher nur im Vergleich zu unbehandelten Personen nachweisbar waren.

Abstract

Intramuscular injection of botulinum toxins (Btx) is intended to weaken dystonic muscles. Concomitant unwanted systemic or autonomic side effects are observed in some cases. In order to measure these autonomic effects, we examined 36 persons, including Btx-treated patients with focal dystonia and hemifacial spasm (n=20) and compared them both before and after Btx injections with healthy controls (n=16) by measuring pupil light reflex, lacrimation and sympathetic skin response. Patients showed significantly prolonged reaction, contraction and redilatation times of the pupil light reflex and reduced lacrimation compared to controls. No significant differences were found upon measuring before and after injection and with respect to the sympathetic skin response. Our results showed systemic autonomic effects of botulinum toxin on the regulation of pupil reflexes and tear secretion in previously Btx-treated patients which surpassed the effects on the muscles and could only be shown in comparison to untreated controls.
