Das anästhesiologische Vorgehen muss bei gefäßchirurgischen Patienten individuell angepasst werden – häufig sind diese durch multiple Komorbiditäten als Risikopatienten einzustufen. Eingriffe an der Arteriacarotis, bei abdominellem Aortenaneurysma oder peripher-chirurgischen Eingriffen bedürfen unterschiedlicher Vorgehensweise bei der Auswahl des Anästhesieverfahrens und des perioperativen Monitorings.

The anaesthesiological management in patients undergoing vascular surgical procedure need an individual approach, because of the high incidence of coexisting diseases with an increased risk of cardiovascular complications. The choice of anaesthesiogical method und perioperative monitoring depends on planned vascular procedures.

Kernaussagen

  • Die Etablierung eines perioperativen Fast-Track-Konzepts kann auch in der Gefäßchirurgie dazu dienen, den perioperativen Stress der Patienten zu reduzieren und die Rekonvaleszenz zu beschleunigen.

  • Eingriffe an der A. carotis: Das perioperative Risiko ist aufgrund der häufig multimorbiden Patienten hoch. Um den Patienten jederzeit neurologisch beurteilen zu können („Biomonitoring“), kann die Operation unter Beachtung von Kontraindikationen in Regionalanästhesie durchgeführt werden. Wenn eine Allgemeinanästhesie notwendig ist, sollte ein Neuromonitoringverfahren (z. B. Nervus medianus SSEP) angewendet werden.

  • Abdominelles Aortenaneurysma: Der Anästhesist muss die verschiedenen operativen Phasen (Clamping und Declamping), in denen es z. T. zu starken hämodynamischen Veränderungen kommt, kennen und vorausschauend handeln. Das Risiko für die Entwicklung von Organfunktionsstörungen ist bei diesem Eingriff hoch.

  • Periphere gefäßchirurgische Eingriffe: Insbesondere bei akuten Gefäßverschlüssen ist der Zustand multimorbider Patienten aufgrund der kurzen Vorbereitungszeit häufig nur insuffizient optimierbar, sodass das perioperative Risiko bei diesen Notfalleingriffen, die den Erhalt der Extremität zum Ziel haben, hoch sein kann.

  • In Abhängigkeit vom Eingriff und vom perioperativen Risiko sollten gefäßchirugische Patienten postoperativ in der Regel auf einer Intermediate-Care- oder ggf. auf einer Intensivstation überwacht werden.
