Kernaussagen

Das perioperative Management in der Gynäkoonkologie ist ein sehr wichtiges Thema für das Outcome der Patientin. Standardisierte Vorgehensweisen sollten etabliert werden, die an das Risikoprofil der Patientinnen und die Operationstechnik (offen, laparoskopisch, robotisch) angepasst werden können.

Abführmaßnahmen sollten generell bei gynäkoonkologischen Patientinnen nicht mehr routinemäßig präoperativ angewendet werden. Eine medikamentöse Thromboembolieprophylaxe sollte präoperativ bevorzugt mit einem NMH eingeleitet und postoperativ bis zu 4 Wochen fortgesetzt werden. Die Antibiotikaprophylaxe, die bei jedem operativen Eingriff bei gynäkoonkologischen Patientinnen zu empfehlen ist, sollte präoperativ verabreicht und bei längerer Operationsdauer wiederholt werden. Postoperativ ist die frühe Nahrungsaufnahme zur Senkung postoperativer Komplikationen, wie Ileussymptomatik und Infektionen, der späten Nahrungsaufnahme vorzuziehen.
