Zusammenfassung
Schmerzen in einer palliativen Situation entstehen oft in Zusammenhang mit einer Tumorerkrankung oder der Behandlung. Der Schmerz kann als rein nozizeptiv oder neuropathisch charakterisiert werden; häufig liegt eine Kombination vor. Der diagnostische „Work-up“ soll Screening-Fragebögen, Schmerzzeichnungen, eine sorgfältige klinische Untersuchung zur Erfassung klinischer Zeichen einer veränderten Schmerzempfindlichkeit sowie bildgebende Verfahren (CT, MRT) und ggf. klassische Elektrophysiologie (Neurografie, SEP) umfassen. Die Tumorschmerztherapie soll nach dem WHO-Stufenschema erfolgen. Aber auch neurologische Erkrankungen wie eine amyotrophe Lateralsklerose (ALS) oder eine fortgeschrittene und schwer verlaufende multiple Skerose (MS) können mit muskulären Schmerzen bei Spastik einhergehen, bei MS auch mit zentral neuropathischen Schmerzen. Weitere neurologische Krankheitsbilder, die auf Palliativstationen behandelt werden und einer Schmerztherapie bedürfen sind weit fortgeschrittene Parkinsonsyndrome und die Chorea Huntington.
Abstract
Pain in a palliative situation is frequently associated with a tumour or its treatment. The pain may be characterised as purely nociceptive or neuropathic, a combination is common. The diagnostic “work-up” should include screening questionnaires, pain drawings, a careful clinical examination for clinical signs of altered pain sensitivity, imaging studies (CT, MRI) and classical electrophysiology (neurography, SEP) in selected cases. Cancer pain management should be based on the WHO treatment ladder. However, other neurological diseases such as amyotrophic lateral sclerosis (ALS) or advanced and clinically severe multiple sclerosis (MS) may also present with muscular or neuropathic pain. Advanced Parkinson’s disease or multiple system atrophy patients also show rigor-associated pain, and Huntington’s disease patients may have pain due to malposition, spasticity and contractures.
