Zusammenfassung

Hintergrund: Hauptsymptom der Patienten mit chronischer Pankreatitis (CP) sind Schmerzen. Zur Genese des Schmerzes bei Patienten mit CP existieren unterschiedliche Hypothesen; eine Hypothese, dass der Schmerz bei Patienten mit CP durch eine Hypertension im Pankreasgangsystem ausgelöst wird, gilt als Argument, um Maßnahmen zur duktalen Dekompression als Therapieoption einzusetzen.

Material und Methoden: Bei 39 Patienten wurden für ein Jahr Pankreasstents zur Schmerztherapie implantiert. Nach einem Jahr erfolgte ein Auslassversuch. Schmerzscores, Schmerzmedikation und Schmerzqualität wurden vor, während und 30 Tage nach Explantation der Stents gemessen sowie nach einer Nachbeobachtungszeit von im Median 44 Monaten.

Ergebnisse: 15 der 39 Patienten litten unter intermittierenden Schmerzen (38,5 %), die übrigen 24 Patienten unter kontinuierlichen Schmerzen. Nach einem Jahr Stenttherapie konnten wir eine insignifikante Verbesserung des Schmerzscores bei 35,9 % der Patienten nachweisen, nach einem Nachbeobachtungszeitraum von insgesamt 44 Monaten waren noch 16,7 % der Patienten schmerzgebessert. Im Zeitraum der Stenttherapie zeigten insbesondere Patienten mit intermittierendem Schmerz eine Reduktion des Schmerzscores (n = 6; 40 %) gegenüber 8 Patienten (33 %) mit kontinuierlichem Schmerz. Im Nachbeobachtungszeitraum wurden insgesamt 27 Patienten operiert. Bei 2 Patienten wurde intraoperativ ein Pankreaskarzinom diagnostiziert.

Zusammenfassung: Da die Entstehung des Schmerzes bei chronischer Pankreatitis ein multifaktorielles Geschehen ist und keinesfalls ausschließlich auf eine Erhöhung des intraduktalen Druckes im Pankreasgang zurückzuführen ist, kann eine ausschließlich auf eine Reduktion des Druckes gerichtete Therapie auch nicht erfolgreich sein.

Abstract

Background: Pain is the leading symptom of patients with chronic pancreatitis. There are various hypothesis about the causes of pain, on hypothesis is that intraductal hypertension within the main pancreatic duct is responsible for pain. Therefore interventions leading to decompression are used to treat painful chronic pancreatitis.

Material and methods: in 39 patients pancreatic stents were inserted to treat pain in CP. After one year stent treatment with scheduled stent exchanges treatment was terminated. Pain scores, pain quality and pain medication were measured before, during and after treatment and during follow up.

Results: 15 of 39 patients had intermittent pain (38.5 %), the remaining 24 patients suffered from continuous pain. After one year stent treatment we observed an insignificant improvement of pain scores in 39.5 % of patients, after follow up of in median 44 months only 16.7 % of patients had pain improvement compared to initial scores. Especially patients suffering from intermittent pain showed an improvement after one year stent treatment (n = 6; 40 %), compared to patients with continuous pain (n = 8; 33 %). However, the results ar not significant different. During follow up 27 patients underwent surgery. In 2 patients pancreatic cancer was diagnosed intraoperatively.

Summary: While pain in chronic pancreatitis is of multifactorial cause not a single procedure is able to successful intervene. Therefore a multidisciplinary approach is required.
