In Abhängigkeit des Ausmaßes der Immunschwäche besteht eine erhöhte Gefährdung für viele Infektionserkrankungen. Daher sind Reisen in die feuchtwarmen Tropen, unter schlechten hygienischen Bedingungen oder in sehr abgelegene Gegenden mit unzureichender medizinischer Versorgung bei ausgeprägtem Immundefekt bedenklich. Impfungen führen bei Immungeschwächten häufig zu einer unzureichenden oder fehlenden Immunantwort. Es können Antikörperbestimmungen und eventuell weitere Auffrischimpfungen notwendig werden. Lebendimpfungen sind potenziell gefährlich und möglicherweise in Abhängigkeit der Schwere des Immundefektes kontraindiziert. Bei Totimpfstoffen ist keine vermehrte Nebenwirkungsrate bekannt. Der akut erkrankte und immungeschwächte Tropenrückkehrer kann infolge fulminanter und/oder atypischer Krankheitsverläufe erhebliche diagnostische Probleme bereiten.

The risk for acquiring infectious diseases correlates to the degree of immunodeficiency. Therefore traveling with severe immunodeficiency is critical under the following circumstances: Destinations in hot and moist climates, traveling under low hygienic standards, or visiting remote areas with reduced medical care. Vaccination of immunodeficient patients may lead to inadequate low or even absent antibody titers. Therefore measurements of these titers and booster-vaccinations may be necessary. Live-vaccines may be dangerous and are often contraindicated according to the immune-defect. Inactivated vaccines do not lead to more side-effects. The immunodeficient patient with acute disease after returning from the tropics may present with atypical and/or severe symptoms, which can confuse the correct diagnosis.
