Ambulante Langzeitentgiftung von Benzodiazepinen bei einem Patienten in Opiatsubstitution

Zusammenfassung

Einleitung:

Benzodiazepinabhängigkeit ist ein häufiges Problem bei Opiatabhängigen in Substitutionstherapie. Als Therapie bei Benzodiazepinabhängigkeit werden Entzugsbehandlungen empfohlen. Neuerdings werden auch Substitutionstherapien diskutiert.

Falldarstellung:

Es wird ein 8-jähriger Krankheitsverlauf eines substituierten Opiatabhängigen mit Benzodiazepinabhängigkeit beschrieben. 2 ambulante und 1 stationärer Entgiftungsversuch misslangen. Eine dritte ambulante Entgiftungstherapie mit einer Dauer von 28 Monaten führte zur Abstinenz. Diese blieb im weiteren Verlauf von 2 Jahren bis auf 3 kurze Rückfälle erhalten.

Schlussfolgerungen:

Wichtige Voraussetzung für das Gelingen einer ambulanten Benzodiazepinentgiftung bei Opiatabhängigen ist eine ausreichende Behandlungsdauer, für die mindestens 1 Monat pro Jahr Abhängigkeit eingeplant werden sollte.

Abstract

Introduction:

Benzodiazepine-dependency is a common problem among opiate addicts in substitution therapy. As therapy for a benzodiazepine-dependency withdrawal treatment is recommended. Recently, substitution therapies are also being discussed.

Case Report:

We describe an 8-year clinical course of a substituted opiate addict with benzodiazepine-dependency. 2 out-patient detoxifications and 1 inpatient attempt failed. A third outpatient detoxification therapy with a duration of 28 months led to abstinence. This situation stayed constant over the course of the following 2 years, with only 3 short relapses during this period.

Conclusions:

An important prerequisite for the success of outpatient benzodiazepine detoxification in opiate addicts is a sufficient duration of treatment, which should be planned for at least 1 month per year of dependency.
