Zusammenfassung

Fragestellung: Es wurde ein systematischer Review gesundheitsökonomischer Evaluationen durchgeführt, die Insulin glargin (GLA) mit Insulindetemir (DET) als Basalkomponente einer intensivierten konventionellen Insulintherapie (ICT) bei Typ-1-Diabetikern vergleichen.

Methodik: Für den Zeitraum vom 1.1.2000 bis 1.12.2009 erfolgte eine systematische Literaturrecherche mittels einschlägiger elektronischer Datenbanken. Die identifizierten Publikationen wurden anhand von vorab definierten Kriterien eingeschlossen.

Ergebnisse: Insgesamt wurden 4 Studien aus 4 Ländern in die Auswertung eingeschlossen. Davon griffen eine Kosten-Nutzwert-Analyse (KNWA) und eine Kosten-Minimierungs-Analyse (KMA) auf die Daten der gleichen randomisierten kontrollierten klinischen Studie zurück. In der KNWA erwies sich die Therapie mit DET gegenüber GLA als preiswerter und effektiver (dominant). In der KMA ergaben sich Einsparungen von € 203 pro Patient und Halbjahr zugunsten von GLA. Eine Routinedatenanalyse unter Verwendung von Daten aus englischen Hausarztpraxen fand unter GLA gegenüber DET signifikant geringere Behandlungskosten von € 159 / Jahr. Eine KMA mit deutschen GKV-Routinedaten zeigte Kostenneutralität hinsichtlich der diabetesabhängigen Gesamtausgaben.

Schlussfolgerungen: In der Bilanz fanden sich zwei ökonomische Evaluationen zugunsten von GLA, eine mit neutralem Ergebnis und eine mit einem Vorteil von DET, je nach Evaluationsmethodik, Datenquellen und länderspezifischen Erstattungsbedingungen.

Abstract

Objective: A systematic review of health economic evaluations comparing glargine (GLA) with detemir (DET) as the basal insulin component in intensified conventional therapy (ICT) in patients with diabetes mellitus type 1 was conducted.

Study design: A systematic literature search was performed between January 1, 2000 and December 1, 2009 in all relevant electronic databases. Retrieved studies were reviewed based on predefined inclusion criteria.

Results: A total of 4 health economic evaluations from 4 different countries fulfilled the requirements comprising 2 modelling studies as well as 2 claims data analyses. One modelling study was performed as a cost-utility analysis, the other evaluations as cost-minimization analyses. The cost-utility analysis identified DET as more effective and less costly than the therapy with GLA over 35 years. One cost-minimization analysis showed savings of € 203 per patient per half-year in favour of GLA. A claims data analysis using data from British general practitioners found significantly lower costs of £ 132 (€ 159) per patient per year for diabetes related prescriptions under GLA. In contrast a cost-minimization analysis with German claims data showed no difference concerning costs of diabetes related prescriptions.

Conclusion: In summary, two evaluations showed an economic advantage of GLA over DET, one was neutral and one was in favour of DET, depending on type of evaluation, data sources and country-specific reimbursement conditions.
