Zusammenfassung
Obwohl die Ruptur des vorderen Kreuzbands eine sehr häufige Verletzung ist, besteht wenig Einigkeit darüber, welche der verschiedenen Behandlungsmethoden die besten Langzeitergebnisse bietet. Viele Hypothesen und Argumente befürworten die eine oder andere Methode.
Diese Literaturarbeit befasste sich mit der grundlegenden Frage, ob die operative oder die konservative Behandlung bessere Langzeiterfolge erzielt. Einige Ergebnisse deuten darauf hin, dass die operativ behandelten Patienten bei der Beurteilung durch verschiedene Kniefragebogen bessere Resultate und eine größere passive/strukturelle Kniestabilität sowie ein geringeres Risiko für spätere Meniskusverletzungen erreichten als die konservativ behandelten. Die funktionellen Wirkungen und das Risiko für Folgeschäden wie Arthrose waren zwischen den Behandlungsgruppen jedoch meistens ausgeglichen, mit einer schwachen Tendenz für ein geringeres Arthroserisiko nach der konservativen Behandlung.
Abstract
Although anterior cruciate ligament rupture is a very common injury there is little agreement about which of the different treatment modalities attains the best long-term results. Many hypotheses and arguments recommend the one or the other method.
This literature review examined the fundamental question whether reconstructive or conservative treatment attains better long-term results. Some outcomes indicate better results of surgically treated patients when assessed using different knee questionnaires as well as passive/structural knee stability and lower risk of future meniscus injury. However, functional results and the risk of subsequent damage such as arthrosis were mostly even between the treatment groups, with slight tendency for a lower arthrosis risk after conservative treatment.
