Zusammenfassung

Einleitung: Ziel dieser systematischen Übersichtsarbeit ist es, die aktuelle wissenschaftliche Evidenz zur Wirksamkeit einer professionell durchgeführten Massage auf Depressivität und Ängstlichkeit darzustellen. Die Übersicht schließt inhaltlich an ältere zusammenfassende Darstellungen oder Metaanalysen an und berücksichtigt klinische Studien bis zum Jahr 2009.

Methoden: Eingeschlossen wurden nur randomisierte klinische Studien (RCT), in welchen die Massage von professionellen Therapeuten ausgeführt wurde. Desweiteren wurden nur RCT berücksichtigt in denen entweder Patienten mit der Primärdiagnose Depression und/oder Angststörung rekrutiert oder Veränderungen in Form von depressiver/ängstlicher Symptomatik als Komorbidität gemessen worden waren. Bevor diese Studien in die weitere Auswertung kamen, wurde eine methodologische Prüfung (PEDro Skala) durchgeführt. Aufgrund der Heterogenität der Studien im Hinblick auf z. B. Studienprotokoll und statistische Kenndaten musste auf eine formale Metaanalyse verzichtet werden, sodass die Ergebnisse deskriptiv dargestellt werden.

Ergebnisse: Von 90 gefundenen Artikeln kamen 22 RCT in die Auswertung. Die methodologische Bewertung mittels der PEDro-Skala ergab, dass 73% (n=16) dem Evidenzgrad A und 27% (n=6) dem Evidenzgrad B zugeordnet werden konnten. Sowohl bei der Behandlung von Depression als auch bei der Behandlung von Angst/Ängstlichkeit wurde überwiegend ein signifikant besseres Ergebnis der Massagetherapie gegenüber den unterschiedlichen Vergleichsgruppen dokumentiert. Die antidepressive Wirksamkeit scheint ausgeprägter als die anxiolytische Wirkung zu sein.

Schlussfolgerung: Es konnte gezeigt werden, dass die Aussagekraft neuerer Massagestudien sich bedeutend verbessert hat. Auch dokumentiert die Übersicht im Vergleich zu früheren Studien, dass die Anwendung der Massagetherapie bei Depression/Angst sich als sinnvoll und effektiv erweist. Für zukünftige Studien erscheint die Fragestellung interessant, ob die Kombination von Massagetherapie und Psychotherapie möglicherweise effektiver ist als Psychotherapie allein oder die Kombination von Psychotherapie und Psychopharmaka.

Abstract

Introduction: The aim of this systematic review is to present recent scientific evidence on the efficacy of a professionally performed manual massage. The overview integrates former reviews or meta-analyses and evaluates clinical studies up to the year 2009.

Methods: Only randomized clinical trials (RCT) were included, in which the massage was performed by professional therapists. Before further evaluating these clinical trials, a systematic methodological test (PEDro scale) was performed. In view of the heterogeneity of the studies, including design and statistical approach, it seemed reasonable to forgo a formal meta-analysis, and to present the results descriptively.

Results: 22 RCTs out of 90 studies were included in the evaluation. The methodological assessment using the PEDro scale showed that 72% (n=16) could be assigned to the level of evidence A, and 18% (n=6) to the level of evidence B. Both in the treatment of depression and in the treatment of anxiety, massage therapy predominantly achieved a significantly better result than the varying control treatments. The antidepressant efficacy appears to be more pronounced than the anxiolytic.

Conclusion: It could be shown that the validity of today's massage trials has considerably improved. The review also demonstrates that the application of massage therapy for depression/anxiety turns out to be useful and effective. For future studies, the question of whether the combination of massage therapy and psychotherapy might be superior than psychotherapy alone or the combination of psychotherapy and psychotropic drugs, would be an interesting research issue.
