In der Prävention des kolorektalen Karzinoms gelten die auch in den Leitlinien enthaltenen
Hinweise auf eine ballaststoffreiche Kost. Vitaminpräparate haben keinen Stellenwert
– Vitamin D könnte eine Ausnahme darstellen. Sekundäre Pflanzenstoffe sollten in größeren
Interventionsstudien geprüft werden.
Für die Misteltherapie wurde eine immunstimulierende Wirkung bisher in vitro und bei
Patienten nachgewiesen. Es fehlt der Beweis einer sich gegen den Tumor richtenden
Wirkung. Die Frage der Verminderung von Nebenwirkungen und Verbesserung der Lebensqualität
kann für die Misteltherapie noch nicht abschließend beantwortet werden. Die Stellung
der Heilpilze ist unklar. Der Einsatz von Pilzpräparaten aus dem außereuropäischen
Bereich ist nicht empfehlenswert. Für eine Reihe von Substanzen der komplementären
Therapie gibt es Hinweise für einen supportiven Nutzen. Einige Präparate können ohne
Bedenken bzgl. Interaktionen eingesetzt werden. Der Einsatz von Antioxidantien zur
Abschwächung von Nebenwirkungen ist dagegen nicht empfehlenswert.
Whereas antioxidative vitamins are not recommended in the prevention of colorectal
cancer, vitamin D could be of importance and the data for phytochemicals should be
confirmed in a clinical interventional trial.
Immunostimulation by mistletoe does not seem to influence the course of the disease
directly but possibly can ameliorate quality of life. The international published
and reviewed data for medical mushrooms are scarce –publications in Asia give promising
results.
Complementary therapies seem to alleviate the sequelae of cancer and cancer treatment,
they are no substitute for a sufficient supportive therapy. They should be used only
if a negative interaction can be excluded.
