Abstract

Two recently presented large controlled trials on telemonitoring in heart failure (HF) patients, the TELE-HF study and the TIM-HF study, failed to show a beneficial effect on total mortality, readmission for any cause, and hospitalization for HF. The neutral results of these two studies indicate the importance of a thorough, independent evaluation of disease-management strategies before adoption. Further studies in selected patient groups may help to identify those HF patients who may benefit from telemonitoring either as „bridge to stability” or as a longterm assistance in their HF management. TELE-HF and TIM-HF indicate that the „human factor” (e. g. personalized communication via a ”nurse-based“ approach) should not be neglected. Furthemore, telemonitoring should not solely allow for monitoring of the health status but also for an individualized optimization of HF therapy. This will not only improve the acceptance but also the success and the prognostic benefit of telemonitoring.
