Zusammenfassung

Eine adäquate Behandlung ausgeprägter Platz­defizite im Unterkiefer erweist sich traditionell als schwierig. Der vorliegende Fallbericht stellt eine erwachsene Patientin mit transversal stark unterentwickelten Kieferbasen und extremen Platzdefiziten in beiden Kiefern vor, bei der nach entsprechender kieferorthopädisch-kieferchirurgischer Planung ein einzeitiges bimaxilläres ­Vorgehen zur transversalen Nachentwicklung durchgeführt wurde. Im Oberkiefer erfolgte eine chirurgisch unterstützte Gaumennahterweiterung, im Unterkiefer eine treppenförmige Osteotomie im Symphysenbereich mit anschließender Distraktion. Der Behandlungszeitraum vom Einsatz der Distraktionsapparaturen bis zur Entbänderung betrug 20 Monate, das Behandlungsergebnis ist stabil. Die Gesichtsästhetik der Patientin hat sich bei zuvor bestehendem Schmalgesicht deutlich verbessert. Nach Ausformung der Zahnbögen kam es zu einer vollständigen Reduktion der Bukkalkorridore. Funktionell wurde eine Opti­mierung der Nasenatmung erzielt, der Kiefergelenkbereich wurde nicht beeinträchtigt. In diesem Fallbericht wird deutlich, dass durch eine gut abgestimmte interdisziplinäre Zusammenarbeit ein reibungsloser Behandlungsablauf und ein klinisch einfaches Handling der medianen Distrak­tion im Unterkiefer möglich ist.

Abstract

An adequate treatment of mandibular transverse deficiencies is a well-known therapeutic chal­lenge. The following case report describes a combined surgical-orthodontic treatment of an adult female patient with severe transverse mandibular and maxillary deficiencies, skeletal deformity with narrow apical bases and extreme dental crowding. The intraoral fixation of tooth-borne expansion devices was followed by a one phase bimaxillary treatment with subsequent appliance activation. In the maxilla, a surgically-assisted ­rapid palatal expansion, in the mandible a cas­caded symphyseal distraction osteogenesis was performed. The treatment period from the fixa­tion of the expanders to the bracket debonding was 20 months, the treatment result is stable. In comparison to the long face configuration at the beginning of treatment, the facial aesthetics of the patient considerably improved. The orthodontic bite correction and teeth alignment result­ed in a complete reduction of the buccal corridors. The nasal breathing was improved, TMJ problems were not observed. The aim of this case report is to show the importance of a well-coordinated inter­disciplinary approach to perform a smooth treatment and an easy clinical handling of a mandibular midline expansion.
