Interdisziplinäre Versorgung am Beispiel des diabetischen Fußsyndroms – Ein Problem der „Kooperation rationaler Egoisten”?

Das diabetische Fußsyndrom ist einfach zu behandeln und bedarf einer guten Zusammenarbeit verschiedener Berufe und medizinischer Fachdisziplinen. Diese Zusammenarbeit ist auch 20 Jahre nach der St–Vinzent–Deklaration organisatorisch verbesserbar. Zur Diagnose gehört die sorgfältige neurologische und angiologische Untersuchung. Therapeutisch sind bei relevanter pAVK (periphere arterielle Verschlusskrankheit) chirurgische infrapopliteale Gefäßrekonstruktionen oder angiologische Gefäßinterventionen notwendig. Die überwältigende Mehrheit der Patienten hat eine Neuropathie. Selbst nach erfolgreicher Behandlung einer pAVK muss auf eine sorgfältige Fußversorgung zur Vermeidung von Druckläsionen geachtet werden.

The diabetic foot syndrome is simple to treat, requires a good co–operation of different occupations and medical technical disciplines. This co–operation is organizational improvable also 20 years after the St. Vincent–declaration. Careful neurological and vascular examination leads to the diagnosis. Therapeutically, surgical reconstructions, balloon angioplasty or stent implantations are necessary in patients with significant PAOD (peripheral arterial occlusive disease). However, the overwhelming majority has a symmetric peripheral neuropathy. Even after successful intervention of concomitant PAD physicians must pay attention to a careful foot supply for the avoidance of mal perforans.
