Klinische Symptomatik, differenzialdiagnostische Schwierigkeiten und Therapieoptionen bei tumorartigen Läsionen der Orbita

Zusammenfassung

Die tumorartigen Läsionen der Orbita gehörten zu den eher seltenen Krankheitsentitäten. In den Jahren von 1996 – 2007 wurden insgesamt 83 Patienten mit Orbitaraumforderungen behandelt, von denen 3 unter tumorartigen Läsionen der Orbita erkrankt waren. Es handelte sich im Einzelnen um ein Riesenzellgranulom, ein eosinophiles Granulom und einen Fall von fibröser Dysplasie. Aufgrund einer zunehmenden Symptomatik und Ausbreitung des Befundes wurden alle drei Patienten chirurgisch behandelt. Ziel dieser Arbeit ist die Darstellung von Klinik, Bildgebung und Behandlung der tumorartigen Läsionen der Augenhöhle sowie auf die Schwierigkeiten des diagnostisch-therapeutischen Vorganges aufmerksam zu machen. Dabei zeigt sich, eine subtile neuroophthalmologische Untersuchung zur Erfassung der Motilitätsstörungen und Bulbusverlagerung, zur Bestimmung des Gesichtsfeldes sowie zur Beurteilung von Visus und Augenhintergrund ist unerlässlich. Eine gute Bildgebung erlaubt zudem, einen optimalen Operationszugang zu planen. Letztlich will diese Übersicht einige Hinweise zur Optimierung des diagnostisch-therapeutischen Konzepts bei tumorartigen Orbitaläsionen geben.

Abstract

Tumour-like lesions are very rare entity among orbital diseases. Between 1996 and 2007, 83 patients with orbital disorders were treated and three of them complained about different tumour-like lesions: giant cell granuloma, eosinophilic granuloma and fibrous dysplasia. Because of the considerable increase of functional disorders and tumour growth, all of the patients underwent surgical treatment. In this paper we describe the clinical symptoms, diagnostics and the methods of therapy for each of the lesions. The results shows that the preoperative neuro-ophthalmological examination as well as radiological imaging are necessary to assess the motility, visual field, vision, eye ball dislocation and to plan the surgical strategy. In terms of this descriptive character of the paper, we hope to submit some relevant information in order to improve the diagnostics and therapeutic procedures for tumour-like lesions of the orbit.
