Les tumeurs malignes du cerveau chez l'enfant

RESUME

L'étude statistique des tumeurs opérées chez l'enfant dans le Service de Neurochirurgie infantile, donne une proportion de 50% de tumeurs malignes, soit: 167 tumeurs. Ces tumeurs sont les plus fréquentes entre 4 et 8 ans.

La localisation de prédilection est la fosse postérieure où se trouvent 71 tumeurs, 39 dans le tronc cérébral. Les localisations sus-tentorielles sont insignifiantes.

La classification par nature histologique donne une proportion importante de glioblastomes (61) et de médulloblastomes (57). Les autres catégories ne donnent qu'un ensemble de 12 tumeurs.

Presque tous les malades ont été irradiés à des doses importantes, mais la survie la plus longue n'a pas dépassé 9 ans.

Plus de deux tiers des opérés sont morts après un an: il ne reste que 30 survivants sur 128 malades contrôlés. La survie décroît ensuite progressivement.

Les traitements médicamenteux antifoliques et antimitosiques ne semblent avoir eu aucune influence sur l'évolution.

Le pronostic reste donc fatal à plus ou moins longue échéance. Il convient de rechercher une modification du milieu empêchant la division cellulaire anormale.

SUMMARY

An investigation of the 167 tumours operated on in the childrens Neurosurgical Service, shows that 50 per cent were malignant. These tumours were the most frequent between 4 and 8 years. The posterior fossa was the commonest site with 71 tumours, and 39 were in the brain stem. Only an insignificant number were supratentorial.

Classification by histological nature showed a significant proportion of glioblastomas (61) and of medulloblastomas (57). All the other types of tumour only totalled 12.

Nearly all the patients were given adequate radiotherapy but the longest period of survival is not more than 9 years.

More than two thirds of the patients were dead after one year, only 30 remaining out of 128 who were followed. The number afterwards decreased progressively.

Medical treatment with folic acid antagonists or antimitotic drugs does not seem to have any effect on their progress.

The prognosis therefore remains fatal sooner or later. It is desirable to look for something that will change the “milieu” and hinder the multiplication of the abnormal cells.

ZUSAMMENFASSUNG

Bei statistischer Untersuchung der im „Service de Neurochirurgie infantile” operierten kindlichen Hirntumoren finden sich 167 maligne Tumoren = 50%. Diese kommen am häufigsten zwischen dem 4. und 8. Lebensjahr vor.

Prädilektionsstellen sind die hintere Schädelgrube mit 71 Tumoren und der Hirnstamm mit 39. Unerheblich ist dagegen das supratentorielle Vorkommen.

Histologisch handelt es sich größtenteils um Glioblastome (61) und Medulloblastome (57). Bei den anderen Arten ergeben sich zusammen nur 12 Tumoren.

Fast alle Patienten wurden mit hohen Dosen nachbestrahlt. Die längste Überlebenszeit betrug aber nicht mehr als 9 Jahre. Mehr als ⅔ der operierten Kinder starben nach 1 Jahr. Von 128 nachkontrollierten Kranken überlebten nur 30. Demnach zeigt die Überlebensquote eine progressive Abnahme.

Die medikamentöse Behandlung mit folsäure- und mitosehemmenden Mitteln scheint überhaupt keinen Einfluß auf den Verlauf gehabt zu haben.

Die Prognose bleibt somit auch weiterhin schlecht. Es wird daher die Forderung erhoben, die pathologische Zellteilung, wenn möglich, durch Änderung des Stoffwechselmilieus zu verhindern.

El estudio estadísticosde tumores operados en los niños en el servicio de neurocirugía infantil dan una proporción de 50% de tumores malignos. En total 167 tumores. El máximo de frecuencia está entre los 4 y 8 años.

La localización preferente es la fosa posterior, donde se encontraron 71 tumores, 39 en el tronco cerebral.

Las localizaciones supratentoriales son insignificantes.

La clasificación según la naturaleza histológica da una proporción importante de glioblastomas (61) y de meduloblastomas (57). Las otras categorías forman un conjunto de 12 tumores.

Casi todos los enfermos han sido irradiados con dosis importantes, pero la supervivencia más larga no ha pasado de nueve años.

Más de dos tercios de los operados han muerto después de un año y no quedan más que treinta supervivientes de 128 enfermos controlados. La supervivencia decrece progresivamente.

Los tratamientos medicamentosos antifólicos y antimitósicos no parecen tener ninguna influencia sobre la evolución.

El pronóstico por consiguiente es fatal a mayor o menor plazo. Es conveniente intentar una modificación del medio impidiendo la división celular anormal.
