Bilateral intracranial carotid aneurysms: Report of a case successfully treated

Summary

A patient with bilateral intracranial carotid artery aneurysms was treated by clipping the neck of one aneurysm intracranially and then by an opposite cervical carotid ligation. This staging of treatment though based upon an erronous clinical diagnosis of which aneurysm had ruptured nevertheless was successful. An important feature in the management of this patient was repetition of arteriography between operative stages and at the completion of treatment, eventually proving both aneurysms had been excluded from the cerebral circulation.

Zusammenfassung

Ein Patient mit beidseitigen intrakraniellen Aneurysmen der Karotis wurde durch Klippen des Aneurysmas der einen Seite und Ligatur der Karotis im Halsbereich der anderen Seite behandelt. Obwohl diese Behandlung auf Grund einer klinischen Fehldiagnose, nach der das Aneurysma rupturiert sein sollte, durchgeführt wurde, so war sie dennoch erfolgreich. Ein wichtiger Gesichtspunkt der Behandlung des Patienten war die Wiederholung der Arteriographie zwischen den beiden operativen Eingriffen und am Ende der Behandlung, um den möglicherweise vorhandenen Ausschluß der Aneurysmen von der zerebralen Zirkulation nachzuweisen.

Résumé

Un malade atteint d'anévrisme intra-cranien bilatéral a été traité d'un côté par clippage du pédicule de l'anévrisme et de l'autre par une ligature de la carotide du cou. Ce traitement par étapes a été courronné de succès, quoique entaché de la difficulté à préciser lequel des deux anévrismes était cause de l'hémorragie. Une deuxième artériographie pratiquée entre la première et la deuxième intervention et une autre après cette dernière, prouvèrent que les deux anévrysmes avaient été exclus de la circulation.

Resumen

Un paciente con aneurismas de arteria carótida intracraneal bilateral fué tratado, mediante clipping del cuello de un aneurisma intracraneal y ligadura consecuente de carótida cervical opuesta. Estos estados de tratamiento, aunque basados sobre un diagnóstico clínico erróneo de dichos aspectos operatorios de aneurisma y tratamiento de complemento, prueban eventualmente que ambos aneurisma han sido excluidos de la circulación cerebral.
