Meningioma within the lateral ventricle

Summary

A case of a 19 year-old girl with meningioma within the left lateral ventricle is described. The calcified mass was visible on straight radiographs. The tumour was successfully removed by transcortical approach in the posterior parietal area. It appeared to be a psammomatous meningioma.

A follow-up study - 10 years after the operation revealed only a slight dysphasia, some right sided sensory and motor impairment, as well as a partial homonymous hemianopia. She has fits, at intervals of four months, despite a proper, though rather irregular medication.

The available literature on intraventricular meningiomas (175 cases), the various signs and symptoms as well as the diagnostic and operative procedure have been discussed.

Zusammenfassung

Es wird der Fall eines 19 Jahre alten Mädchens mit Meningiom im linken lateralen Ventrikel beschrieben. Der kalzifizierte Tumor war im einfachen Röntgenbild sichtbar. Er wurde durch einen transkortikalen Zugang in der hinteren Parietalregion erfolgreich entfernt. Es handelte sich um ein psammomatöses Meningeom.

10 Jahre nach der Operation waren lediglich eine leichte Dysphasie und geringe rechtsseitige sensible und motorische Ausfallserscheinungen sowie eine leichte homonyme Hemianopsie nachweisbar. Die Patientin hat Anfälle in Abständen von 4 Monaten, obwohl sie ausreichend eingestellt war, wohl infolge unregelmäßiger Medikamenteneinnahme.

Die greifbare Literatur über intraventrikuläre Meningeome (175 Fälle), die verschiedenen Zeichen und Symptome sowie die diagnostischen und operativen Maßnahmen werden besprochen.

Observation d'un méningiome du ventricule latéral gauche chez une jeune fille de 19 ans. Les radiographies simples montraient une calcification. La tumeur, extirpée avec succès par voie transcorticale pariétale postérieure, était un psammome. 10 ans après l'opération on ne relevait qu'une légère dysphasie et de discrets troubles sensorio-moteurs et hémianopiques droits. Malgré une médication adéquate mais irrégulièrement suivie, des crises nerveuses apparaissent à des intervalles de 4 mois. Partant de la littérature disponible sur les méningiomes intraventriculaires (175 cas), les symptômes variés, le diagnostic et les mesures opératoires sont discutées.

Resumen

Se comunical el caso de una chica de 19 años con meningioma en el ventrículo lateral izquierdo. El tumor calcificado se pudo observar en la radiografía. Por vía transcortical en la región parietal posterior se exstirpó con éxito. Se trataba de un meningioma samomatoso.

10 años después la operación existía sólo una ligera disfasia y algunos síntomas de déficit motor y sensible en e lado derecho así como una ligera hemianopsia homónima. La paciente tiene ataques cada 4 meses, probablemente debidos a irregularidades en la toma de los medicamentos, aunque se le había ajustado a una dosis de mantenimiento suficiente.

Se discuten la literatura sobre meningioma intraventriculares (175 casos), los distintos síntomas así como las medidas terapéuticas y diagnósticas.
