Summary
A 30 year old white female with hydrocephalus and progressive spasticity involving all four extremities secondary to a calcified aneurysm presumably of the great vein of Galen is described. The intracranial calcification was first seen on radiological examination when the patient was 5 years old.
The radiographic findings were again verified at age 30. Almost complete surgical removal of the calcified mass was obtained with the exception of a small piece of the capsule adherent to the brain stem. There was significant improvement in the neurologic status of the patient with decrease in the severity of the spasticity.
The entire literature on vein of Galen malformations is reviewed with special emphasis on the diagnosis from plain skull films.
Zusammenfassung
Der Fall einer 30 Jahre alten Frau mit Hydrocephalus und fortschreitender Tetraspastik als Folge eines verkalkten Aneurysma der Vena magna Galeni wird beschrieben.
Die intrakranielle Verkalkung wurde erstmals anläßlich einer Röntgenuntersuchung der Patientin im Alter von 5 Jahren gefunden. Diese radiologischen Befunde wurden im Alter von 30 Jahren erneut bestätigt. Es konnte die nahezu komplette Ausräumung der kalzifizierten Masse mit Ausnahme eines kleinen Stücks der Kapsel, die am Hirnstamm anhaftete, durchgeführt werden. Es ergab sich eine signifikante Verbesserung im neurologischen Status der Patientin mit Rückgang der Spastik.
Es wird die wesentliche Literatur über die Mißbildungen der Vena Galeni dargestellt unter besonderer Berücksichtigung der Diagnose auf Grund von Röntgenaufnahmen des Schädels.
Résumé
Cas d'une femme de 30 ans atteinte d'une hydrocéphalie et d'une tétraspasticité progressive consécutives à une anévrysme de la grande veine de Galien. La calcification intracranienne a été découverte la première fois à l'âge de 5 ans à l'occasion d'un examen radiologique. Cette constation a été confirmée à nouveau à l'âge de 30 ans. On a pu exciser presque complètement cette masse calcifiée à l'exclusion d'une petite partie de la capsule adhérente au tronc cérébral. Il s'en est suivi une amélioration significative de l'état neurologique avec rétrocession de la spasticité. On fait état de la littérature portant sur les malformations de la veine de Galien en insistant sur le diagnostic basé sur les radiographies du crâne.
