Zusammenfassung

Die Prognose der Tumoren in der hinteren Schädelgrube bei Kindern und Jugendlichen ist nicht allein abhängig von dem histologischen Bild des Tumors, sondern zumindest ebenso von dem Sitz in oder in der Nachbarschaft der Liquorabführenden Wege, der Größe des Tumors und der durch späte Diagnostik bereits entstandenen Vorschädigungen im Bereich der vegetativen Funktionsareale des Kleinhirns, des Hirnstammes und des Mittelhirns. Der Langzeitverlauf von 147 Tumoren der hinteren Schädelgrube zwischen dem 1. und 20. Lebensjahr wurde verfolgt. Neben Spongioblastomen, Zysten und anderen gutartigen Prozessen haben wir die Ependymome und Plexuspapillome wegen ihres ungünstigen Sitzes zu den bedingt gutartigen gerechnet, dementsprechend auch die relativ ungünstige Operationsund Gesamtmortalität.

Auch der histologische Befund der gutartigen Prozesse scheint von dem Sitz nicht unabhängig zu sein. Die histologische Kontrolle der Spongioblastome zeigte vor allem mit der zusätzlichen Silberfärbung zur Darstellung der astrozytären Fortsätze, daß im Gegensatz zu den üblichen Lokalisationen in den Kleinhirnhemisphären und dem hier normalen bis völlig ausdifferenzierten Bild beim Wachstum dieser Tumoren im Bereich des Hirnstamms, der Brücke oder auch des Mesencephalons eher undifferenzierte polymorphe Wachstumstendenzen vorherrschen. Neben der Frühdiagnose sollte auch die unterschiedliche histologische Klassifikation für die Beurteilung der Tumorentwicklung im Kindes- und Jugendalter mehr Beachtung finden.

Summary

The prognosis of tumours of the posterior fossa in childhood and adolescense is not only dependant upon the histology of the tumour but also upon its situation, especially in relation to the C.S.F. pathways, the size of the tumour and any damage that might have arisen owing to a late diagnosis involving the cerebellum, brain stem and mid brain. The long term follow up of 147 tumours of the posterior fossa between the first and twentieth years of life recounted. Besides spongioblastomas, cysts and other benign processes, we have included as relatively benign: ependymomas and choroid plexus papillomas and because of their situation have consequently accepted the relatively poor operative and overall mortality.

Furthermore the histological picture of the benign processes does not seem to be independant of the location of the tumour. The histology of spongioblastomas using techniques of silver staining for astrocytic processes revealed that these tumours tended towards undifferentiated growth phenomena. This finding was in contradistinction to those lesions usually situated in the region of the cerebellar lobes and to the histological picture which might vary from the normal to total undifferentiation in the growth of tumours in the region of the brain stem, pons and mid brain. Besides early diagnosis histological classification should find a greater place in the evaluation of tumour development affecting this group of cases.

Le pronostic des tumeurs bénignes du cervelet et du tronc cérébral chez l'enfant.

Le pronostic des tumeurs de la fosse postérieure chez l'enfant et l'adolescent dépend non seulement de l'image histologique, mais au moins autant du siège de la tumeur sur les voies d'écoulement du 1cr ou à leur voisinage, de sa taille ou des lésions déjà constituées dans les aires fonctionelles du cervelet, du tronc cérébral ou du mésencéphale du fait d'un diagnostic trop tardif. L'évolution de longue durée de 147 tumeurs de la fosse postérieure répartie sur les âges de 1 à 20 ans a été suivie. Nous avons rangé les épendymomes et les papillomes avec les spongioblastomes, les kystes et d'autres tumeurs bénignes quoique leur localisation souvent défavorable pour l'intervention pouvait aggraver les résultats opératoires et la mortalité. L'aspect histologique des processus n'apparait pas indépendant de leur localisation: L'histologie du spongioblastome montrait, en particulier sous imprégnation argentique pour mettre en évidence les prolongements des astrocytes, que l'image d'une évolution complètement différenciée lors de la localisation usuelle dans l'hémisphère cérébelleux, montrait une tendance vers la prolifération indifférenciée lors de la localisation dans la protubérance et le mésencéphale.

L'attention mériterait d'être portée davantage sur un diagnostic précoce et une classification histologique différentielle pour apprécier le développement de ces tumeurs chez l'enfant et l'adolescent.
