Adipositasprävention: Von der wissenschaftlichen Erkenntnis zur Umsetzung

Zusammenfassung

Weltweit hat sich die Prävalenz der Adipositas unter Erwachsenen seit 1990 von 8 auf 18 % mehr als verdoppelt, bis 2050 wird eine weitere Zunahme erwartet. Auch in Deutschland steigt die Prävalenz kontinuierlich. Dies zeigt, dass bisherige Präventionsbemühungen unzureichend bleiben. Die Evidenz zu wirksamen Maßnahmen wird u. a. in der im Herbst 2024 veröffentlichten S3-Leitlinie „Prävention und Therapie der Adipositas“ zusammengefasst. In der Legislaturperiode 2021–2025 hatte sich die Bundesregierung ambitionierte Ziele gesetzt, aber nur wenige Maßnahmen umgesetzt. Der Koalitionsvertrag 2025–2029 enthält nur einzelne konkrete Aussagen zur Adipositasprävention. Geplant ist u. a., 1 Mrd. Euro für die Modernisierung und Sanierung von Sportstätten bereitzustellen. Studien zeigen, dass evidenzbasierte Präventionsmaßnahmen – besonders schulbasierte und solche mit Positivanreizen (z. B. Steuerbefreiung für Obst und Gemüse) – breite Unterstützung finden. Gleichzeitig wird die Debatte um Ernährung oft als polarisierend erlebt. Um gesellschaftliche Veränderungen zu ermöglichen, braucht es eine respektvolle Kommunikation und inklusive Narrative, die an gemeinsame Werte anknüpfen.

Abstract

Worldwide, the prevalence of obesity in adults has more than doubled since 1990—from 8% to 18%—and is projected to nearly double again by 2050. Germany has also seen a continuous increase in prevalence across the population, highlighting the inadequacy of prevention efforts. The evidence and recommendations for effective measures are well established. They are summarized in the updated German evidence- and consensus-based guideline on the prevention and treatment of obesity, published in autumn 2024. During the 2021–2025 legislative period, the German federal government set ambitious goals for obesity prevention, yet only a few measures were successfully implemented. The coalition agreement for the 2025–2029 legislative period contains only few concrete commitments. Several recent publications show that evidence-based prevention measures are supported by a majority of the population in Germany. This particularly applies to school-based measures, and policy packages that include positive incentives, such as tax breaks for fruit and vegetables. The public discourse on nutrition is often perceived as divisive. Respectful and inclusive communication is essential, along with unifying narratives that appeal to a broad range of political values and align with current societal priorities.
