Dental care for rare diseases: Case reports of hereditary sensory and autonomic neuropathy and severe chronic neutropenia
Zusammenfassung
Die zahnmedizinische Versorgung von Patienten mit hereditären sensorischen und autonomen Neuropathien (HSAN: „hereditary sensory and autonomic neuropathy“) oder schwerer chronischer Neutropenie erfordert eine enge interdisziplinäre Zusammenarbeit und eine individuelle Behandlungsplanung. HSAN-Patienten leiden unter einer fehlenden Schmerzempfindung, was zu unbemerkten Verletzungen, nicht heilenden Ulzerationen und ausgeprägter Karies führen kann. Dies erfordert spezielle Präventionsstrategien, regelmäßige Kontrolluntersuchungen und ggf. restaurative oder chirurgische Eingriffe. Patienten mit schwerer chronischer Neutropenie zeigen aufgrund der reduzierten Immunabwehr ein erhöhtes Risiko für aggressive parodontale Erkrankungen, die rechtzeitig diagnostiziert und behandelt werden müssen. Die Fallbeispiele verdeutlichen, dass eine frühzeitige Behandlung essenziell ist, um orale Komplikationen zu minimieren und die langfristige Zahngesundheit zu erhalten.
Abstract
The dental care of patients with hereditary sensory and autonomic neuropathies (HSAN) or severe chronic neutropenia requires close interdisciplinary collaboration and individualised treatment strategies. HSAN patients suffer from a lack of pain perception, which can lead to unnoticed injuries, non-healing ulcers and pronounced caries. This requires special preventive measures, regular check-ups and, if necessary, restorative or surgical interventions. Patients with severe chronic neutropenia show an increased risk of aggressive periodontal disease due to reduced immune defences, which must be diagnosed and treated at an early stage. The case studies illustrate that early interdisciplinary care is essential in order to minimise oral complications and maintain long-term dental health.
