Zusammenfassung
Hintergrund Zur Stärkung von AAPV fehlt es oftmals an Konkretisierung der Handlungsfelder für
hausärztliche Praxisteams. Im Projekt „ALLPRAX“ wurde der Frage nachgegangen, welche
Maßnahmen aus Sicht von Hausärzten und Medizinischen Fachangestellten das Potenzial
haben, die AAPV in ihren jeweiligen Praxen zu verbessern.
Methode Acht Praxisteams wählten in jeweils einem Workshop anhand eines in einer vorherigen
Projektphase entwickelten Maßnahmenkatalogs, geeignete Maßnahmen für eine Erprobung
und Weiterentwicklung in ihrer eigenen Praxis aus.
Ergebnisse Die Praxisteams erachteten 35 aus 120 Maßnahmen für geeignet, die Palliativversorgung
in ihrem Zuständigkeitsbereich zu verbessern. Am häufigsten wurden Maßnahmen zur vorausschauenden
Versorgung von Patienten, Patienten- und Angehörigen-Beratung sowie Bildungsmaßnahmen
für die Praxisteams ausgewählt.
Schlussfolgerung Zur Weiterentwicklung von AAPV in Hausarztpraxen bedarf es eines umfangreichen und
zugleich differenzierten Interventionspakets.
Abstract
Background To strengthen PPC, there is often a lack of concretisation of the fields of action
for GP practice teams. In the “ALLPRAX” project, the question was investigated which
specific measures, from the perspective of GPs and medical assistants, have the potential
to improve palliative care in their respective practices.
Methods In one workshops each, eight GP practice teams selected suitable measures on the
basis of a catalog of measures developed in a previous project phase in order to test
and further develop them in their own practices.
Results The GP practice teams considered 35 of 120 measures to be suitable for improving
palliative care in their area of responsibility. The measures most frequently selected
were those for anticipatory patient care, patient and family counselling, and further
education for the GP practice teams.
Conclusion The further development of PPC in GP practices requires a comprehensive and differentiated
intervention package.
